Hello doctor,

My son has a positive ANA and Scl 70. I just do not get it. I am in shock as I am sure anyone else would be. Does this mean for sure he has scleroderma? He has no symptoms. What should I do?

Positive ANA (anti-nuclear antibody) suggestive of some form of autoimmune disorder or connective tissue disorder. Anti-Scl (scleroderma) positivity suggestive of scleroderma. But provide me your report as an attachment for giving more comment as I want to know reference range of anti-Scl 70 (anti-topoisomerase antibody) for that laboratory. Thank you and take care. Anti-nuclear positivity should be evaluated further. Here, in attached report (attachment removed to protect patient identity) no other autoimmune disease workup has been done. According to physical examination different workup can be planned. If anti-Scl is positive, then the diagnosis can be scleroderma. Anti-centromere testing also can be done for further workup in scleroderma. Following are different causes for positive ANA: Rheumatoid arthritis, for which RA (rheumatoid arthritis) factor and anti-CCP (cyclic citrullinated peptide) can be done, SLE (systemic lupus erythematosus), for which anti-dsDNA (double-stranded DNA) investigation can be done, scleroderma, for which anti-Scl and anticentromere testing can be done, polymyositis, dermatomyositis , sjogren syndrome, for which anti-RoLa (Arabidopsis thaliana) can be done, Vasculitis, autoimmune thyroiditis, inflammatory bowel disease, etc., can be the other causes. Sometimes, in infectious mononucleosis, ANA can be false positive. So, if needed for further workup autoimmune cell panel investigations can be done in a laboratory where you have investigated. For physical examination consult nearby rheumatologist or pediatrician. Thank you and take care. Regards.

Thank you for your response. I did send you the autoimmune results. What do you think about Scl 70, 1.7? It is not too high, right? Maybe that makes a difference?

I could not find anti-Scl 70 antibody investigation in your attached report. In that report only ANA is positive. Anti-Scl 70 has not been mentioned. Kindly post anti-Scl 70 if has been done. I hope this helps. Thank you and take care. Regards.
