Hello doctor,

I am a 26-year-old female. For a few days, I have been having anal pain. Initially, I thought it was due to piles; however, when I read on the internet, it shows anal fistula. I am very scared. Can you help me with the best possible solution?

I understand your concern and would be happy to help. A clinical examination will help to get an accurate diagnosis. Do you have any previous history of perianal surgery for a perianal abscess? Fistulas can occur after such surgeries. Also, is there any painful defecation or a history of constipation? How many liters of water do you consume in a day? Are you a vegetarian or a non-vegetarian? Have you undergone any previous surgeries? Do you take any medications? Can you give a detailed history and share an image for further diagnosis?

Thank you for the response.

I am a non-vegetarian. I drink around one liter of water daily, and there is no history of diseases, surgeries, or medications. Earlier I used to get hard stools with blood; however, now I do not have that complaint.

I understand your concern. I have seen the shared images (attachments removed to protect the patient's identity). As per your history, you have an anal fissure due to a lower intake of water. It can also be a skin tag if you feel something palpable on the skin.

I suggest the below treatment plan:

  • Syrup Lactihep 15 ml (Lactitol and Benzoic Acid) once daily at night.
  • Lactifiber powder (Isphaghula powder and Lactitol Monohydrate) two teaspoons in a glass of water twice daily.
  • Cream Fidonal (Lidocaine 1.5 % w/w and Nifedipine 0.3 % w/w) for local application.
  • Tablet Lanol ER (Paracetamol 650 mg) twice daily.
  • Tablet Pantoprazole 40 mg daily once half an hour before breakfast for ten days.

I also suggest a sitz bath in which you have to add lukewarm water to a tub, add 10 ml of Betadine (Povidone-Iodine 10 % w/w), and sit in the tub for 15 minutes daily, twice or thrice a day; avoid non-vegetarian food for three months, take around four liters of oral fluids, a high fiber diet, 700 ml of water and a brisk walk of 15 minutes in the morning. Also, avoid straining in the washroom to prevent the development of piles.
