I have an anal fissure. The fissure first appeared about five months ago after passing a hard stool. I had bleeding only one time.

Your history is suggestive of a chronic anal fissure, likely triggered by hard stool about five months ago. One episode of bleeding with hard stool is common with an anal fissure.

Typical symptoms seen in an anal fissure. You may experience

  1. Pain or burning while passing stool.

  2. Small amounts of fresh red bleeding on the tissue or stool.

  3. Tightness or spasm around the anus.

  4. Fear of passing stool because of pain.

There are several measures to help improve the condition and promote healing. You should do the following.

  1. Keep the stool soft with a high-fiber diet, fruits, vegetables, and plenty of water.

  2. Avoid constipation and straining.

  3. Take a warm sitz bath for 10 to 15 minutes after passing stool and two to three times daily.

  4. You may use a stool softener such as Lactulose if the stools are hard.

  5. Topical ointments such as Lignocaine with Diltiazem or Nitroglycerin are commonly prescribed to relax the muscle and help heal the fissure.

You should seek examination by a general surgeon if any of the following occur:

  1. Pain continues for more than six to eight weeks.

  2. Repeated bleeding occurs.

  3. There is swelling, pus, fever, or severe pain.

  4. You feel a lump or skin tag.

  5. Constipation is persistent.

Most fissures heal with conservative treatment if the stool remains soft consistently.
