Hello doctor,

I noticed my daughter of 4 years and 5 months is itching in her ano. She told it was getting hard when she wanted to pass stools. I noticed a mild inflammation in her ano and a mild crack. Please suggest a cream or medicine for her. I have Sucral Ano Cream. Can I apply? Please help us.

I can understand your concerns.

I have seen the picture that you have attached (attachment removed to protect patient identity). There appears to be a cut over perianal region due to hard stools.

In my opinion, I suggest the child should be given:

  1. Syrup Oflomac-M (a combination of Ofloxacin and Metronidazole) 0.13 fluid ounce three times a day for three days.
  2. Syrup Lactulose 0.13 fluid ounce three times a day for three days.
  3. Syrup Bandy Plus (Ivermectin and Albendazole) two teaspoons stat once at night for deworming single time only.
  4. Apply Sucral ano cream (Sucralfate, Lidocaine, Metronidazole) and two times a day for one week.
  5. Give more water to the child so that constipation and hard stools do not occur.

Consult your specialist doctor, discuss with him or her and start taking the medicines with their consent.

Is Oflomac-M really necessary? My daughter started constipating even after a single dose of antibiotic. Please advice.

The antibiotic is definitely needed because infection can also lead to constipation and hard stools which can result in anal fissure and cut at perianal region. Oflomac-M syrup does not result in constipation. The causes of constipation are if the child drinks less water or if the child does not eat all types of food. Antibiotic and constipation are not at all related. I suggest you start the antibiotic.

Same symptoms don't mean you have the same problem. Consult a doctor now!
