How to effectively manage cardiac arrhythmias?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I need a few of my ECGs reviewed from my health records. I am experiencing sharp chest pain that spreads across my chest, shortness of breath, and a rapid or fluttering heartbeat. My pulse rate fluctuates between extremely low and extremely high. Currently, I have been unable to lower my heart rate below 120 BPM for several hours.

Please help.

Hello,

Welcome to icliniq.com.

It is not just about relieving your chest pain. A proper diagnosis and treatment plan must be initiated. A beta blocker is effective in lowering a fast heart rate, but in the case of bradycardia, it could further reduce the heart rate. To determine if there is any sinus pause, Holter monitoring and echocardiography need to be performed, along with free T4 (thyroxine), T3 (triiodothyronine), and TSH (thyroid-stimulating hormone) tests. Sometimes, thyroid function issues can cause an irregular heartbeat. So, do not worry—complete the tests and begin treatment accordingly. Please visit the hospital for the tests.

Regards.

Patient's Query

Hello doctor,

Thank you for the reply.

I appreciate your time and responses. My main priority for tonight is to have the ECGs I attached interpreted. Could you please interpret them? I know your time is valuable, and I truly appreciate your help.

Hello,

Welcome back to icliniq.com.

You are experiencing cardiac arrhythmias. As you have already mentioned that you have idiopathic cardiomyopathy, I have suggested two things to evaluate both conditions and plan proper treatment. Due to your fast heart rate (tachycardia) and very slow heart rate (bradycardia), which is known as tachybrady arrhythmia, this may occur in the presence of an accessory pathway in the heart.

Therefore, I recommend Holter monitoring. For idiopathic cardiomyopathy, you may need to undergo echocardiography to assess your heart's function in more detail. Do not worry, and make an appointment with your cardiologist. Always remember, you can solve any problem if you remain calm in stressful situations.

Regards.
