Can atopic dermatitis lead to red patches on the face?

Hi doctor,

I am a 40-year-old female, single, diabetic on Metformin; I started to have skin lesions on my face about a month ago, patches of raised, red, sore, scaling skin on my face; I had it before at my hairline, and near the ear area. I would like to confirm this seborrheic dermatitis and what to do about it. Please help.

I understand your concern. It is an allergic rash (atopic dermatitis). I suggest applying 1 % hydrocortisone cream or gel at night for seven days. Apply Nadibact cream (Nadifloxacin) or 1 % Clindamycin gel during the daytime for ten days. Wash face with gentle Cetaphil cleanser (Aqua, Glycerine, Panthenol, and Sodium benzoate) twice daily for 15 days. Take Tablet Antoxid HC (Beta-carotene, Copper, Manganese, Selenium, and Zinc Sulphate) once daily after breakfast for 30 days. Avoid scratching and rubbing of lesions.

I suggest applying Kojic cream (Kojic acid, Lactokine fluid, Axeloglicina (Potassium azeloyl digylycinate-PAD), or Eukroma cream (Hydroquinone) at night for 21 days. Take Tablet Limcee 500 mg (Vitamin C and Ascorbic acid) once daily after breakfast for 30 days. Apply photostable sunscreen daily from 9 AM to 3 PM for two months. Get adequate sleep. Take proper nutrition. Drink seven to eight glasses of water daily. Avoid direct sunlight. Use sunglasses. Use hats and sun protection measures. Avoid scratching and rubbing.

I noticed that after I stopped Hydrocortisone cream (I used it once at night for seven days), the lesions started to become swollen and red again after a few days; what shall I do about it? I am only using Cetaphil face wash and eczema moisturizer, no new acne lesions, but the same lesions of eczema I showed before just came back after stopping Hydrocortisone.

It is best to stop Hydrocortisone. I suggest taking Tablet Doxycycline 100 mg once a day after food for 15 days. Avoid direct sunlight while on Doxycycline. Use photostable sunscreen daily from 9 A.M to 3 P.M. for two months. Apply Acnesol gel (Clindamycin) twice daily for 15 days. Take Tablet Zincovit (multivitamin tablet) once a day after food for 30 days. Avoid scratching and rubbing of lesions. Moisturize skin daily with Venusia max cream (Aloe butter, Shea butter, Mango butter, Cocoa butter, and Glycerine). Follow up after two weeks.
