Can discontinuation of anti-fungal cream cause recurrence of balanitis?

Hello doctor,

I have been suffering from balanitis since last month. I am using an antifungal cream to avoid it. Till the time I use antifungal cream all rashes or infections disappear, but once I stop applying the cream all symptoms reoccur.

Symptoms are given below:

  1. Red skin around the head of the penis.
  2. Itchiness after one to two days of red skin.

Balanitis is an inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. Predisposing factors include poor hygiene and over-washing, use of over-the-counter medications, and non-retraction of the foreskin. May be associated with diabetes, and candida fungal infections.

Investigations to be done are given below:

1. Blood sugar.

2. KOH (potassium, oxygen, and hydrogen) amount for candidiasis.

Treatment:

1. Gentle retraction of the foreskin daily.

2. Soak in warm water to clean the penis and foreskin.

3. Use a combination of Fusidic acid (Fusidin) and Clotrimazole (Cruex) twice daily for one month. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

4. If balanitis does not respond to topical therapy then you can take Ketoconazole (Nizoral) 200 mg twice daily for two weeks. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

Since you were using steroid-containing cream that is why it recurring. In recurrent cases, 1 % Pimecrolimus (Elidel) cream should be used instead of steroids. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

Thank you for the reply.

I have had this problem for the past two years. I have done oral sex (mouthing) from the first day to five days continuously. After a burning sensation, this red spot came.

The following problems appear:

1. Dry mouth.

2. Frequently thirsty.

3. Frequent urination.

4. Burning sensation.

5. Sometimes feeling of itchiness.

If I put in antifungal cream then it is no more itchy.

Thank you for the reply.

All these tests I had taken four months ago but am struggling for two years.

Following are the tests and their results:

  1. HIV test:- Non-reactive.
  2. VDRL:- Non-reactive.

I request you to please guide me in the right way. For the last three months, I took ayurvedic treatment. With tablets RG (Reishi Gano) and GL (Ganocelium) there was no reappearance of this infection. For the last 10 days, I have not taken RG (Reishi Gano) and GL (Ganocelium) and the infection reappeared. Please try to explain what is happening to my health.

You can use Clotrimazole (Lotrimin) and also Fusidic acid (Fusidin) cream twice daily for one month and take a tablet of Fluconazole (Diflucan) 150 mg once a week for four weeks. Consult your specialist doctor, discuss with him or her and take the medicines with their consent.

You should get your partner treated, may be the source of infection is there only.

Hello doctor,

Thank you for the reply.

Now I am applying a combination of Clotrimazole and Beclometasone (Candid B) cream and Fluconazole (Diflucan) tablet twice a day. The infection disappears within three to four days and if I stop taking the medicine after 15 days it again reappears. Otherwise while sleeping, semen comes out and I do not clean it for more than five hours after it appears. While cleaning time of penis, I had seen some white layer if I clean it normally. If I do not clean my penis one day, the white layer reappears within two days. If this infection happens then the white layer appears slightly more. When there is an infection, the penis gets shrunk (smaller size) and I also get to see erectile dysfunction. After the infection disappears, the penis gets back to normal.

Firstly, you should stop applying Candid B (Clotrimazole and Beclometasone) because it also contains steroids. Consult your specialist doctor, discuss with him or her and take the medicines with their consent. Secondly, steroid causes recurrence, you just start Clotrimazole (Lotrimin) plus Fusidic acid (Fusidin) cream twice daily for one month and take a tablet of Fluconazole (Diflucan) 150 mg once a week for four weeks. Consult your specialist doctor, discuss with him or her and take the medicines with their consent. Also, get your partner treated and get your blood sugar levels done.

I want to know why is it happening again and again. Is there any reason? How should I wash my penis? Can I use soap?

It seems there are two reasons for the reoccurrence given below:

1. Due to the use of steroid-containing cream Candid B (Clotrimazole and Beclometasone).

2. One source of infection is most likely your partner, your partner should be treated simultaneously.

You should wash your penile area with normal tap water, no need to apply any soap, you may apply any antiseptic liquid over this area.
