My penis and surrounding foreskin are infected and covered with white spots. It has been itching for two days. I have not taken any medication until now.

I understand your concern. It is balanitis with a secondary bacterial infection. Apply Fucidin (Fusidic Acid) cream twice daily for seven days. Take tablet Zifi (Cefixime) 200 mg twice daily after food for five days. Wash genitals with Episoft (Cetearyl Alcohol) cleanser for one month. Maintain proper genital hygiene and avoid using harsh soaps or fragrance-based lotions. Avoid scratching and rubbing lesions, and keep the area clean and dry. Keep checking your sugar levels and avoid injury or friction in that area. Get a urine routine culture microscopy and complete blood count with STI (sexually transmitted infections) panel tests.

Thank you for the reply. I hope you have gone through the attached image. I do have a black mark on my penis. It has been there since my birth. Will it hurt, or will I have any side effects in the future?
