Patient's Query

Hello doctor,

I am 18 years old and have been experiencing symptoms of balanitis for the past two to three months. It is painful, and I have noticed that using certain soaps makes it worse. I also get rashes on the head of my penis. Currently, I only take anti-allergy tablets when necessary. Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

It appears that you may be experiencing balanitis, which is inflammation of the glans penis. When the inflammation involves the foreskin and prepuce, it is referred to as balanoposthitis. Here are some important points to consider:

  1. Predisposing factors: Balanitis can be caused or exacerbated by poor hygiene, overwashing, the use of over-the-counter medications, and the non retraction of the foreskin.

  2. Possible associations: Balanitis may be associated with conditions such as diabetes and candidal fungal infections.

  3. Investigations: It is advisable to undergo certain investigations, including checking blood sugar levels and performing a KOH (mount test to detect candidiasis.

Treatment options include:

  1. Do gentle retraction of the foreskin daily and soak the penis and foreskin in warm water to ensure proper hygiene.

  2. Consider using the combination of Fusidic acid and Clotrimazole twice daily for one month. If balanitis does not respond to topical therapy, consult your general physician for consideration of Ketoconazole 200 mg twice daily for two weeks.

Follow-Up: To determine the most suitable treatment plan and assess progress, it is essential to get a review with pictures for better evaluation.

Thank you.
