Hello doctor,

I am a 22-year-old shy male with a 160-pound weight and 5.9-foot height. When I poop, I see blood on toilet paper, I am afraid of having cancer. Please help.

I have read the query, and I can understand your concern.

I am a gastroenterologist and will be addressing your concerns. As per your history and the attached image (attachment removed to protect the patient's identity), your case appears to be a grade three hemorrhoid (hemorrhoids prolapse and usually require manual reduction). As per the protocol, stages one to three are managed conservatively, while surgery is done on stage four hemorrhoids.

Suggestions for conservative management are-

  1. Start taking Syrup Lactulose 10 to 15ml once daily before going to bed. It is one of the most frequently used laxatives, tolerated very well by patients in the long term with minimal side effects. Do not just use any stool softener.

  2. Secondly, instead of using Preparation H (Benzoic acid, Butylated hydroxyanisole, and corn oil), use Calcium Dobesilate, Hydrocortisone, Lignocaine, and Zinc combination. It will provide much better relief, marketed by the brand name Ruticool.

  3. Thirdly, request your gastroenterologist prescribe oral Calcium Dobesilate and Doxium therapy for three months.

Consult a specialist doctor, talk to them, and then take the medication.
