Patient's Query

Hello doctor,

I experienced swelling and bruising on my penis, which resolved after four days. However, I have noticed a loss of sensitivity along the edge of the penis. Masturbation now takes five to ten minutes for ejaculation, and there's lingering pain in the bruised area. Morning erections are absent, and the penis feels fatigued from nocturnal erections. I can only achieve erections in the afternoon and evening, and standing erections are no longer possible. There is mild pain when blood flows into the penis. I have an upcoming appointment with a urologist. What should I expect during the consultation? Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I can understand your concern.

To better understand your situation, I would appreciate your responses to a few questions:

  1. Are you diabetic or hypertensive?

  2. Have you undergone any surgeries in the past?

  3. When did you first notice issues with your erections?

  4. Can you recall how the injury to your penis occurred? Was it during sexual activity or masturbation?

  5. Can you achieve a normal erection suitable for intercourse at any point during the day?

  6. Have you experienced any difficulties with urination, such as increased frequency, urgency, straining, or a sense of incomplete bladder emptying?

  7. Are you currently taking any medications?

It is likely that the urologist will conduct a thorough history-taking during your visit and may recommend blood tests and ultrasound to further investigate the situation.

I hope this information will help you.

Thank you.

Patient's Query

Hello doctor,

Thank you for your reply.

  1. I have been taking Indapamide for hypertension for the past five years, and fortunately, it has not had any adverse effects on my ability to achieve and maintain an erection.

  2. Approximately six months ago, I experienced an incident where I woke up with a swollen prepuce. While the swelling subsided relatively quickly, it has not completely healed. I still experience mild discomfort, and it appears to affect the blood flow to my penis, causing pain and a decrease in blood circulation.

  3. The injury was likely caused by exposure to an industrial cleaner liquid or a bite from a small venomous spider. Despite the incident, I can still achieve erections in the afternoon and evening, and I am comfortable in various positions such as lying on my back, on the side, and sitting. However, I have noticed difficulty in obtaining erections in the morning since the incident. Standing erections also seem more challenging, possibly attributed to the aging process. I maintain a regular gym routine, including increased focus on leg exercises, but I have noticed a sensation in my legs during standing erections.

  4. I do not experience any issues with urination.

  5. There has been a change in the moisture level between the shaft and the head of my penis. Previously, it was slightly moist due to being uncircumcised, but now it feels drier and less sensitive. Achieving erections takes more time, and I suspect it may be related to exposure to industrial food cleaners.

  6. Ten years ago, I underwent surgery for an inguinal hernia, and while the discomfort has lessened, there are still occasional sensations of pain within the penis.

  7. Additionally, I have observed a slight bending of the head of the penis when it is empty. These concerns have prompted me to seek advice or guidance on addressing these issues.

Thank you.

Hello,

Welcome back to icliniq.com.

Firstly, if you can achieve a normal erection suitable for intercourse at any time of the day, it indicates that there is no functional issue affecting your ability to attain an erection.

Functional problems typically prevent the attainment of an erection altogether. Various factors, such as anxiety, your perception of a normal erection, and stress, can influence the quality of erections.

It is worth noting that individuals who regularly practice prone masturbation may perceive weaker erections in positions other than lying face down.

There does not seem to be any serious cause for concern based on your description. However, considering your history of hypertension, I recommend undergoing some tests to ensure overall health and address any potential concerns related to erections.

The suggested tests include the following:

  1. Urine routine.

  2. Renal function test.

  3. HBA1C.

  4. Lipid Profile.

  5. Uroflowmetry.

  6. Ultrasound KUB and penile doppler.

  7. Semen for culture and sensitivity.

I recommend consulting with your GP or Urologist to request these tests and provide further insights. Once the results are available, you can either revert to me or discuss them with your healthcare provider for appropriate guidance.

I hope this information will help you.

Thank you.
