Hi doctor,
Exactly one week ago, I did the second treatment of Permethrin 5 % cream for scabies. This week, I have little pimples on my penis, and my scrotum is itchy. I am aware this is likely a post-treatment reaction. How can I get rid of these pimples?

I understand how upsetting it is to see new bumps and itching in such a sensitive area. What you are describing is very common following successful scabies therapy. One week after the second application of Permethrin 5 % often small nodules or pimple-like bumps on the penis or scrotum are due to post-scabies reaction (post-scabetic nodules or eczema).
This is because your immune system is still reacting to the dead mites and their debris even after the infestation has been treated. The itching may last for two to four weeks or longer even with appropriate therapy. However, just to be sure, it would be very helpful if you could upload clear photos of the affected areas. Postscabetic nodules.
Permethrin irritant dermatitis.
Folliculitis.
Persistent or reinfestation (less common if treatment was done correctly)
In the meantime you can start with basic supportive care. Apply a bland emollient, like petroleum jelly or a fragrance-free moisturizer, twice daily to calm irritation and assist in healing the skin barrier. Do not scratch if possible, as this can aggravate inflammation and lead to secondary infection.
Loose cotton underwear will help reduce the friction and sweating in this area which can aggravate the itching. Routine reapplication is usually not necessary if you applied Permethrin correctly, covering the whole body from the neck down, leaving it on for eight to 12 hours, repeating the application after seven days and ensuring close contacts were treated.
Re-treatment is considered only in the presence of definite signs of active scabies, such as new burrows, spreading rash or worsening nocturnal pruritus.
