Hello doctor,

There are some red bumps on my penis head accompanied by itching. Two sore patches are present on both sides of the frenulum. Currently, there is one small patch on the tip of the penis that looks like an ulcer. The frenulum also has a red dot and a short red line. This has been there for more than a week and I am worried that it could be a sexually transmitted disease or HIV, though I tested negative for HIV. I have experienced such red bumps on the penis head previously following sex with my partner. But, after a week it will disappear. But, now it has not been resolved. I am not taking any medications. Please help.

I have seen your query and I can understand your concern. From the pictures (attachments removed to protect the patient's identity) shared by you, it seems that you have developed genital herpes. Genital herpes, which is the most common cause of genital ulcerations, accounts for 20 to 50 percent of all genital ulcers. After the usual incubation period of three to 14 days, small, grouped vesicles make their appearance in about four to six days and soon ulcerate. Do you have any pain, or dysuria (pain while urination) as your main symptoms? Is there any painful and tender lymphadenopathy?

Primary genital herpes (first episode) may last for 18 to 21 days. 80 % of the primary genital herpes infections are due to HSV-2 (human simplex virus), which recur more frequently than those due to HSV-1. I suggest you take the tablet Acyclovir 400 mg three times a day and use 5 % Acyclovir ointment twice daily for five days after consulting your GP. Also, kindly check your blood sugar levels, do a VDRL (Venereal disease research laboratory) test, HBsAg (Hepatitis B surface antigen), and anti-HCV (antibody against Hepatitis C virus) test to rule out causes of immunosuppression.
