Hi doctor,

I have been dealing with what I thought was a UTI for three months. It started with a fever and got worse one month back. I rubbed my genitals with a transgender girl and used her semen to masturbate until I cummed. We did not have sex. She did give me oral sex. I got prescribed Ciprofloxacin 500 mg for five days. Symptoms persisted. I got Doxycycline for five days and Azithromycin one pill the next day, and the symptoms did not go away. Another doctor suggested Doxycycline 100 mg for 10 or 14 days. After that, I still felt the symptoms. All blood work and urine tests came back negative. I checked everything and did three urine tests.

The symptoms did not bother me too much until last month. I did notice I had a slight bruise-looking thing next to my meatus. I did not think anything of it. Then, in the middle of last month, my penis and balls shriveled up like I just got out of a cold shower which lasted until I started using Clotrimazole 1 %. Another doctor told me I had a yeast infection or balanitis from what I described. I had the urge to urinate more and dribble. I am circumcised. I had a slight red rash that would only show after a hot shower right above my meatus at the tip. My libido and everything took a dive two months back, and I have been depressed and trying to figure out what is wrong with me. It was hard for me to get an erection. I have not had sex or masturbated since the beginning of last month. I think the Clotrimazole helped at first, but then it made my meatus inflamed. I spoke to a urologist next in person. He looked at my penis and just said it looked slightly inflamed prescribed me Nystatin and Triamcinolone acetonide cream for ten days. It seemed to work, but I stopped using it after five days. My penis glans looked wrinkly, and my meatus was swollen and red and dry.

Currently, I am taking Fluconazole one 150 mg and three pills. I am not sure if it is helping, as I am on day six. I also went to a dermatologist to check, and she said she did not see anything except dryness and told me to put Vaseline on my penis twice a day until gone. I can get hard again, and the shriveled penis and testicles have been back to normal. I still have the urge to pee slightly, but I think it is due to drinking more water. I feel like I have to pee sometimes and only pee a little. My penis head will sometimes be irritated when I have to pee, and once I pee, it will go away. I feel a burning sensation at the tip when I hold my pee and not while peeing. My meatus feels inflamed sometimes and is sensitive. Not sure if it is just dry from the creams. I do have dribbling after I pee but not too bad. For the past week or so, I have been waking up once at like 3 AM to pee. It is the tip and head of my penis that are always irritated. Kindly help.

I understand your concern. To me, those symptoms seem to be chronic prostatitis, and it has two types, one is with bacterial infection, and the other is without infection. Diagnosis is by semen culture, which is done in any laboratory. If it is positive bacteria, then I suggest taking a choice of antibiotics and using them for four weeks. If it is negative, then I suggest you can use Septran DS (Sulfamethoxazole and Trimethoprim) 980 mg twice daily for four weeks in addition to Tamsulosin 0.4 mg once at night. But remember, semen culture is before starting antibiotics. I also suggest you get screened for sexually transmitted infections by blood tests like hepatitis B and C, syphilis, herpes simplex, and HIV (human immunodeficiency virus).

Thank you for the reply. I got tested for everything you mentioned, and it came back negative. I did a PSA test, too, and it was 1.64 ng/ml. So I need to go see a urologist get this test done? I was hoping it was not prostatitis. Kindly help.

Great to hear that STIs (sexually transmitted infections) are clear. I suggest you go to the laboratory and request the test, or ask your urologist to give you the request. The symptoms you described seem to be prostatitis.
