Hello doctor,

I am 60 years old and no longer have my periods. I had an ultrasound pelvic transverse and they found a heterogeneous and thickened endometrium, with a large cystic structure adjacent to the right ovary measuring 4.7cm may represent a right para ovarian cyst or hydrosalpinx. Findings of the uterus are normal in size with a standard appearance of the myometrium. The uterus measures 5.5x2.9x3.9cm and the endometria strip is heterogenous in appearance and has a thickness of 8mm. There is a possible small 3mm cyst in the endometrium. The left ovary was not seen, while the right ovary measures 2.8x2.2x2.4cm. Additionally, there is a large 4.7x3.9cm cystic structure adjacent to the right ovary. No free fluid is present within the pelvis. I had an MRI (magnetic resonance imaging) pelvis contrast, which revealed a.3.3 x4.4x3.5cm right adnexal simple cyst. Due to my postmenopausal status, there were also findings of cholelithiasis (gallstones) without evidence of cholecystitis (inflammation of the gallbladder). The MRI also showed mild hepatic steatosis (fatty liver) and sigmoid diverticulosis without evidence of diverticulitis. Findings: the liver is normal in size and homogeneous in signal intensity. There is a 1.7 cm stone in the gallbladder. Could this indicate cancer? Can you provide me with more information about these findings??

I can understand your concern. Endometrial thickness of more than 4 mm in post-menopausal women is of significance. Though you have mentioned no post-menopausal bleeding, it is better to get an endometrial biopsy done. Any cyst in postmenopausal women should be evaluated. You can discuss with your doctor about CA (cancer antigen) 125 testing. Though you have mentioned that it is a simple cyst, getting the test done will give a clue whether it is benign or malignant.

We cannot jump to conclusions if cancer is present unless diagnosed. Discuss with your gynecologist and get back if you have other questions.
