Hello doctor

I had chest pain and had to rush into an emergency sometimes two months back. In my ECG, the findings shows probable left atrial enlargement and Abnormal R-wave profession with early transition. However, the main cause of concern was ruled out as a gall bladder stone stuck in the neck. Today I got an HRCT scan done. I was diagnosed as COVID positive on the first of last month and negative on the nineteenth of last month. However, when I lie down to sleep, one side of my nose was getting blocked (mostly the left side), and my SpO2 is between 93 to 96 with a low-grade fever of 99 to 100 degree Fahrenheit morning and evening. From the past three days, I again developed a slight cough and chest pain which I thought might be because of a gall bladder issue, so anyhow went in for HRCT. HRCT came back as normal. However, the report mentioned mild cardiomegaly and small volume aortopulmonary, right paratracheal lymph nodes noted. I am still having slight chest pain towards my left side now, and I feel that my body is not getting much oxygen or blood oxygen from the past year. I am attaching all investigation reports for review. What might be the cause for mild cardiomegaly and small volume aortopulmonary? Is the pain that I am experiencing now interrelate? Does it require immediate treatment or any further investigations? Currently, I am also facing allergic conjunctivitis in both eyes. I am only using eye drops. Please guide me.

I have gone through your reports (attachments removed to protect the patient's identity). Your ECG (electrocardiography) was normal, and your current investigations are also normal. All inflammatory markers and HRCT (high resolution computed tomography) did not show any evidence of active COVID or any other infection. Cardiomegaly on CT (computed tomography) is not a reliable finding. It is frequently a falsely positive finding. So at this age with normal ECG, it is unlikely to be a significant finding. However, since a CT scan creates doubt, you should undergo an echo to confirm that everything is normal. Normal echo would rule out any possibility of cardiomegaly. Also, small lymph node enlargement is not an issue. It was there in your previous ECG as well. It is due to past infection, and again not a worrisome finding. So both these findings are not significant on HRCT. However, it is better to go for an echo to be sure. Cardiac chest pain occurs classically on exertion and relieves with rest. Pain at rest associated with coughing is usually not related to the heart. I hope this helps you. Please get back if you have any doubts.
