Hello doctor,

I have left side chest, upper back, and shoulder pain for the past four years. I have undergone tests. Maximum time, I feel drowsy. I am not able to sit in the office as I feel pain and drowsy. I am feeling so anxious as my pain is on my left side. Cardia tests, ECG, and echo showed normal. Please suggest how to overcome dizziness and pain. I am 37-years-old. I have no work tension. But my left side pain is making big trouble in all my day-to-day activities. I am attaching my recent blood reports. Kindly help me.

Please answer the following to know more. Do you have any family history of heart diseases? Do you have high blood pressure or other risk factors like smoking or alcohol? I have gone through your reports (attachment removed to protect the patient's privacy), and there are a few derangements like, Low vitamin D: I suggest supplements like Tablet Uprise D3 60,000 units once a week for eight weeks with milk, Slightly low ferritin: You should have plenty of green leafy vegetables and other iron-rich foods like dates, beetroot, etc, Deranged lipid profile especially elevated triglycerides, Slightly elevated uric acid: Healthy lifestyle and reducing intake of uric acid-rich food like meat (if non-veg), Deranged liver enzymes, Slightly elevated fasting blood sugar. Rest reports are fine. Your symptoms are multifactorial. Some components are physical, and some might be psychological. Physical components are contributed by low Vitamin D (which can be corrected by having the above supplements) and being overweight. To be precise, your symptoms are likely due to Vitamin D deficiency and probably cervical problems (which might be related to low Vitamin D and obesity). They are unlikely to be related to the heart. Heart-related pain classically occurs on exertion like walking or climbing stairs and relieves with rest. However, additionally, I suggest you take a treadmill test (TMT) to reliably rule out the cardiac cause. Other components in your symptoms might be psychological, like anxiety or depression, lack of interest in the work (although there is no stress), etc. You should also focus on regular exercises, meditation, and yoga, which can help in psychological upliftment. For derangements in lipid profile, liver enzymes derangements, you should focus on a healthy lifestyle like avoiding fatty, oily, and high-calorie foods. Have regular exercises, have a light dinner, and try to achieve some weight loss. Avoid alcohol intake if there is any consumption. You should repeat the lipid profile and liver function test after four to six weeks to see the response. If there is a response, then you need to continue with a healthy lifestyle, or else you might need medicines for the same. I suggest you undergo additional tests like an ultrasound of the liver to confirm fatty liver or rule out other abnormalities. Also, I suggest you undergo viral markers like hepatitis B surface antigen (HBsAg) and antibodies against hepatitis C virus (Anti-HCV) for these infections as a cause of liver enzymes derangements. Hello doctor,

Thank you for the reply. There is no history of heart disease in my family. I do not have any habits like drinking or smoking. Because of pain, I frequently take painkillers. Fatty liver was present when I underwent a test six months ago. I used to do work with pain and lightheadedness only (with dizziness). If I am dizzy, I used to walk and stretch my chest and left hand and continue my work. The main concern is discomfort in the chest, upper back, and left hand.

As I mentioned above, the cause appears to be non-cardiac, especially cervical. Besides supplements, you will require lifestyle modifications like discipline in diet, exercise, and probably some nourishment for the mind as well.
