Hello doctor,

Four months ago, I faced severe pain in the lower abdomen area with a discharge from my penis twice. After that, inside the penis, there was itching and a burning sensation. I cannot hold urine for more than 2 hours while sitting. Sometimes I can see transparent pus on my penis in the morning. Sometimes I feel pain in the scrotum. I feel good in some situations, like walking and standing, as I do not feel any symptoms, and in some situations, like jumping and running, these symptoms feel worse. I am not married and have had no sexual encounters. I am a non-alcoholic and non-smoker. I have done a blood test, normal urine analysis, and normal urine culture, and no growth was found. I have done ultrasonography of the abdomen with PVR, and no abnormalities were seen. My urine flow test was normal, but there was a low flow when holding the urine for a long time. I need to find out what the disease is. Please help me, doctor. Currently, I am taking Azithral 500 mg, Exena 50 mg, and Alfoo 10 mg, but these medicines have not given any good results. Earlier I was using Doxylag 100 mg and Buscopan 10 mg, and these gave good results.

Having gone through your symptoms, it seems you are having a problem known as chronic prostatitis. This is an infection of the prostate gland, and it has a tendency to reoccurrence and increase and decrease in severity. Your earlier medication of Doxycycline will have a good effect because it works well in chronic prostatitis. Though this problem is not a danger to life, it does become very bothersome. My advice for you is to get a prostate examination done by a urologist to confirm the diagnosis. Intermittent prostatic massage by a urologist may help to relieve the problem. You will need to continue medication like Doxycycline or Nitrofurantoin for a long period of time under the guidance of a local urologist. This problem gets aggravated by anxiety, stress, and lack of sleep, so if you have these problems, you will need to take care of them also.

No, there is no specific test for prostatitis. It is diagnosed on the basis of symptoms and physical examination. Few tests like PSA (prostate-specific antigen) and transrectal ultrasound may help in diagnosis, but no test is confirmatory. All tests need to assess together to make the diagnosis.
