Hello doctor,

I am a 28-year-old female and I have a 4-year-old daughter who is turning five the next month. I am seeking to see a fertility specialist because I have been having unprotected sex in hopes to get pregnant and it has been five years and I have no second baby. A year or two ago I saw urgent care because I was bleeding for about two consecutive months.

She then prescribed me pills which she explained were some hormones to slow down the flow. That worked for a while then period went back to normal. I then tried the Mirena IUD for about two weeks in which I had constant bleeding and had it removed. I still have not gotten pregnant. I am not sure why? I am not on any medications and my weight has not changed at all. Please help.

The irregular bleeding mostly is due to some sudden hormonal imbalance. As you feel, your weight has not changed a lot then that mostly is not the reason for this. But still, try to be active and keep trying to lose few kilograms. Your expected body weight is between 52 to 57 kg which I know is impossible. But try to reduce at least four to five kilograms. Hope thethyroid hormone and prolactin hormone abnormalities have been ruled out.

As your problem is of secondary infertility, the first thing to do is to confirm the fallopian tube patency. This is the tube where the egg meets the sperm to form an embryo. It can never be seen on the scan. You will have to do an HSG (hysterosalpingogram) or laproscopy. Once confirmed as patent, your chances of pregnancy will be very high. Your doctor can plan for ovulation induction with hormonal tablets and injections. This will definitely improve your chances of pregnancy. Since you have had a pregnancy, you will surely have a pregnancyagain.
