Hello doctor,

I have noticed some black spots, or black pimples, for a while near my penis. I wonder what is that? I did my STD test, and it came out negative. The last time I had intercourse was almost three years ago. I did my test after six months, and everything came out negative then. After a year and two years ago, I did the test again and came out negative. I wonder what is that? It is not itchy, swollen, and there is no burn or pain. I have attached the images.

As per the history and images provided from your end (attachment removed to protect the patient's identity), it appears to be genital warts or condyloma acuminata. Genital warts are a viral infection caused by papillomavirus, which is considered to be sexually transmitted or through direct skin contact with an infected person. As you have mentioned, with a history of having physical contact three years back and having done a sexually transmitted disease (STD) test, genital warts may take weeks but sometimes even months or years to appear and may remain asymptomatic. It is diagnosed with complete blood test or biopsy if required. I would suggest you take a tablet of Zinc acetate twice daily in the morning and night after food for one month. Consult your specialist doctor, discuss with them and take the medicines with their consent. Electrocautery is an outpatient procedure done by a dermatologist where the lesions are gently burned and later on heal. For this procedure, or for cryotherapy, where the lesions are frozen and eventually fall off and heal, you may need to consult with a dermatologist. Avoid unprotected sex, notify any future sexual partners about the same, and advise your sexual partner to get treated. Also, kindly consult a doctor for human papillomavirus (HPV) vaccination to prevent reinfections with a different strain. Genital warts may be recurrent and therefore taking adequate precautions during intimacy is essential. Kindly consult a dermatologist for further evaluation and the treatments mentioned above. Hello doctor,

Thanks for the quick response doctor. Is it curable? Because based on online reports, HPV goes away within a year or two. The immune system fights off HPV. Will I ever get it again if it is removed? Am I at high risk or something? Do I need to be worried? Can I have unprotected sex after the warts are removed? Please let me know as I got nervous when you said warts.

I want to assure you that it may be removed via electro-cautery by a dermatologist. Once you have gotten rid of it, make sure you use Zinc acetate tablets that I have prescribed which help in immune development and fighting viral infections. Consult your specialist doctor, discuss with them and take the medicines with their consent. You may also take multivitamins regularly for two months and take a break of one month after that and continue with the vitamins again after one month for a period of two months, and so on. It can help strengthen your immunity. Regarding unprotected sexual intercourse, I would suggest you always use physical contraceptive methods and not follow unprotected sexual intercourse. In addition, human papillomavirus (HPV) infections sometimes may come on and off and vary from individual to individual. Therefore, it is always good to be careful and have periodic testing by a doctor.
