Patient's Query
Hi doctor,
I had unprotected sexual intercourse on day one of my period. The penis was rubbed against my vagina, but we did not have penetration. I was worried due to the precum. So, I took the Levonorgestrel emergency contraceptive pill on day two of my period within 13 hours.
Kindly help.
Hello,
Welcome to icliniq.com.
I understand your concern.
As you have taken the Levonorgestrel emergency contraceptive pill early on day two of your cycle, you may or may not get withdrawal bleeding. The chances of pregnancy are minimal, as you are in your safer period with regular cycles. If you do not have withdrawal bleeding, then get a serum beta human chorionic gonadotropin test done after eight days to rule out pregnancy. If it is negative, wait for your menses, which may be delayed this time.
I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.
Thank you.
Patient's Query
Hello doctor,
Thank you for your reply.
Today is the sixth day of my period, and I am bleeding in a lesser amount than a regular period. I have not had the bleeding for a long time.
Kindly help.
Hello,
Welcome back to icliniq.com.
I understand your concern.
As you took the Levonorgestrel emergency contraceptive pill on day two of your menses, progesterone withdrawal will start by day four, as the Levonorgestrel emergency contraceptive pill is a high dose of progesterone, which causes bleeding until the effect of high-dose progesterone exposure wears off in the next 36 to 48 hours. Withdrawal bleeding can be added to normal endometrial shedding, and thus, the bleeding can be prolonged. But when the flow is reduced and is not changing more than two to three pads a day, the bleeding can stop in the next two to three days.
The tiredness, weakness, and sleepiness are due to the progesterone effect of the Levonorgestrel emergency contraceptive pill, which can cause bloating, often constipation, mild nausea, and heaviness in the head (common progestogenic symptoms). These can be alleviated or controlled by increasing your daily water intake to 0.5 to 0.8 gallons a day and keeping yourself well hydrated. The most common side effect of the Levonorgestrel emergency contraceptive pill is irregularity in one or two menstrual cycles, which can often be heavy. So, it is advised to stop taking the Levonorgestrel emergency contraceptive pill for the next three months. Practice barrier contraception or switch to low-dose oral contraceptive pills for the next three months.

Hello doctor,
Thank you for the reply.
I had the sexual act again. Still, I am bleeding.
Kindly help.
Hello,
Welcome back to icliniq.com.
I understand your concern.
It is the withdrawal bleed, which has combined with your menses. There is no way to get regular periods for the next cycle naturally. I advise you to eat healthily, drink plenty of water, and keep yourself de-stressed. However, you can take low-dose oral contraceptive pills for a month or two. On the day you stop your pack, within the next three to five days, you will get your menses. Pregnancy is unlikely to happen. The flow will end naturally in the next few days.

Patient's Query
Hello doctor,
Even today, I have a little bit of bleeding, and the bleeding was light brownish. I am drinking lots of water, as you suggested. For how many days will I have this bleeding?
Kindly help.
Hello,
Welcome back to icliniq.com.
I understand your concern.
The bleeding generally lasts for four to five days. If the bleeding has reduced to spotting with light brown discharge, it will stop within the next 1 to 2 days. You have to be patient. I would like you to be relaxed. When the Levonorgestrel emergency contraceptive pill is taken, this pattern of bleeding is expected in the early proliferative phase of the cycle, where the endometrium is shed and rebuilt. This process is delayed by three to four days. You can expect your bleeding pattern accordingly.

Patient's Query
Hello doctor,
My bleeding stopped yesterday, but there is some white discharge. Sometimes, I usually get it. My feeling of sleepiness has also gone. Is this kind of discharge normal?
Kindly help.
Hello,
Welcome back to icliniq.com.
I understand your concern.
It is good to hear that your bleeding has stopped. It is a normal cervical discharge in response to rising estradiol levels during the proliferative phase. The discharge may be thin and homogeneous in type. It is seen in the mid-cycle preovulatory phase for two to three days. It is normal physiology.

Patient's Query
Hello doctor,
Today is the 14th day of my cycle, and as mentioned earlier, I took the Levonorgestrel emergency contraceptive pill on day two. Today, I got some white spotting. Is this normal?
Hello,
Welcome back to icliniq.com.
I understand your concern.
The white discharge or spotting by day 14, the mid-cycle spotting, which is non-itchy and non-foul-smelling, is cervical mucus, which is physiological, indicating that you are nearing ovulation time. It is not a post-Levonorgestrel emergency contraceptive pill effect. So, in short, this is normal to experience in the mid-cycle.
The only possible side effect of post-Levonorgestrel emergency contraceptive pill intake that you may experience is a possible delay in the next menses by 7 to 10 days, and it may be heavier than normal. However, the pattern may vary from woman to woman. Hope you are practicing barrier contraception this cycle?

Hello doctor,
Yes, doctor. The discharge is creamy in texture and sticky. It is not clear in color and not stretchy, and it has caused milk-white spotting. I hope this is normal.
Kindly help.
Hello,
Welcome back to icliniq.com.
I understand your concern.
Yes, that is normal for the mid-cycle. This may last for two to three days, and once you ovulate, this will decrease when natural progesterone levels rise in the second half of the cycle. No itching and no smell rule out any vaginal infection.
You may use plain water alone or an alkaline vaginal wash to clean yourself. Do not use any soap locally in or around the vagina. That actually causes a change in vaginal flora and can increase discharge. Hence, soaps should be avoided completely, and preferably, plain water washes are recommended.

Patient's Query
Hello doctor,
Today is the 17th day of my cycle. I generally have two muffins in the morning since I have to go to work and then have lunch. I often feel discomfort in the stomach, salivate more, and hear gurgling sounds in the stomach. Since the last half a month or so, this has been happening more often.
Kindly help.
Hello,
Welcome back to icliniq.com.
I understand your concern.
The symptoms of bloating and gurgling you experience in the afternoon usually indicate dyspepsia and are often due to delayed gastric emptying, which can occur as a side effect of progesterone withdrawal. However, this is typically seen 3 to 4 days after intake and not after many days.
Many young women become anxious and assume that vomiting or bloating is always related to pregnancy, which is not necessarily true. So, there is no need to worry on that front. However, if you want conclusive documentary proof, you may get a serum beta human chorionic gonadotropin test done. If the value is less than 3 mIU/mL, pregnancy is conclusively ruled out, as this is the gold standard test.
The symptoms you are experiencing are more suggestive of dyspepsia and hunger pangs, where increased salivation and sudden epigastric cramps are common. You should have small, frequent snacks between meals after your muffin breakfast. A muffin alone is not a healthy breakfast, and you should include milk with cereals for a more balanced meal.
As advised earlier, please maintain adequate daily water intake, as this helps reduce most progestogenic side effects during this cycle. The white discharge should have been resolved by now. The blood test is only to relieve pregnancy anxiety. Alternatively, you may opt for a home pregnancy test using a test kit.
I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.
Thank you.
