Hello doctor,

I am a 19-year-old male and I weigh 138 pounds. My height is 5 feet and 8 inches. I have been coughing for four years. I often spit out a lot of sputum (yellow or white), a few times with small amounts of blood. Recently, I have had frequent chest pain. I often feel tired and have a loss of appetite. After physical exertion, for example, jogging, I experience chest pain. At the same time, my lung capacity does not seem to have decreased. I do not take any medicines. Please help.

I feel very sorry about your medical condition. Do not worry. Based on the symptoms that you have described, I suppose you have developed pulmonary tuberculosis. Acute chest syndrome needs to be ruled out. Do you have any history of night sweats, weight loss, or difficulty breathing? Please do a chest X-ray, sputum MCS (sputum culture), and Mantoux test. I suggest you take the following:

  1. Tablet Isosorbide mononitrate plus aspirin (Vasoprin) 75mg orally every day.
  2. Capsule Rifampicin 600 mg orally thrice a day for three months.
  3. Tablet Isoniazid 300 mg orally every day.
  4. Tablet Ketoconazole 200 mg orally every day.
  5. Tablet Acyclovir 800 mg orally every day.
  6. If your symptoms persist, schedule another appointment for a follow-up.
