Hi doctor,

My doctor wants to perform a flexible cystoscopy in his office to investigate hematuria. I have no bladder control due to a spinal injury; it voids spontaneously at low volume and low pressure. I am worried I will leak around the scope when they fill. I am sure they have dealt with this before, but I would like to know what to expect. What usually happens in such a case?

I understand your concern.

Please be assured that if done properly, a flexible cystoscopy will not cause you any problems. It is a routine office procedure done day in and day out. As far as leaking around the scope, it generally does not happen because the doctor performing your cystoscopy will have his finger occluding any space around the scope. In such a scenario you probably would not leak. Even if you do, it is nothing to be embarrassed or concerned about. A flexible cystoscopy is a relatively painless and quick procedure, much like having a catheter inserted if you have experienced that feeling. I hope this puts your mind at ease. He also mentioned evaluating spasticity during the cystoscopy (not as part of urodynamic study). How might spasticity be checked during cystoscopy

Cystoscopy is used to evaluate duration and severity of your issue by looking at the degree of bladder trabeculations and also checking at what capacity of filling does your bladder start to hurt and contract. A UDE study is needed for proper evaluation however the cystoscopy may give a faint idea as well. Please let me know if I can help you with anything else?
