How can I manage my dark patches and white spots on my skin?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have long-standing dark patches with some white spots on my neck, beard line, and upper back for more than one year. I also have dark pigmentation in both armpits. There is no itching, pain, scaling, discharge, or spread. Only marks are present.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

Based on your description, long-standing dark patches with small white spots over the neck, beard line, and upper back, along with dark pigmentation in both armpits, present for more than one year without itching, pain, scaling, discharge, or spread, suggest a chronic pigmentary condition (long-term skin disorders) rather than an active infection.

In my opinion, the conditions you may suffer from a skin condition. The skin conditions are described below.

  1. Acanthosis nigricans (a skin condition characterized by dark, velvety, thickened patches, usually appearing in body folds like the neck, armpits, and groin).

  2. Post-inflammatory or frictional hyperpigmentation (a common, often temporary, darkening of the skin that occurs after an injury).

  3. Pigmentary mosaic or pityriasis alba–like changes (benign, self-limiting skin disorder).

  4. Less commonly, tinea versicolor (a non-contagious, superficial fungal skin infection).

Darkening in the armpits is most often related to friction, shaving, deodorant reactions, insulin resistance, or weight-related factors, rather than any dangerous disease.

To make an accurate diagnosis, I need clear photographs of the affected areas. Please share close-up photos of the neck, beard line, upper back, and armpits, taken in natural light if possible, without filters or flash. Visual assessment helps differentiate between pigmentary disorders, fungal causes, and metabolic or friction-related changes, which can appear very similar without examination.

For the armpits, you may use Kojivit plus (Vitamin E Acetate) cream once daily at night. It contains Glycolic acid and Kojic acid, which can help lighten the pigmentation.

In the meantime, the absence of symptoms such as itching, pain, discharge, or rapid spread is a good sign and usually indicates a non-serious condition. Please follow these general care measures:

  1. Avoid excessive scrubbing and harsh soaps.

  2. Do not use lemon or home remedies.

  3. Avoid tight collars and repeated friction.

  4. Use a gentle cleanser.

  5. Keep the affected areas dry.

  6. Avoid frequent shaving or the use of strong deodorants in the armpits until evaluation.

Once the diagnosis is confirmed, I will be able to provide an exact treatment plan, as each possible condition requires a different approach.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
