Hello doctor,

My problem is both my underarms are black in color and have a bad smell when they sweat. I did not use any medicine for my underarms before. My face is oily type with acne spot marks. I have consulted you and you suggested that I use Yaclin facewash, Benzoyl peroxide, AC-N, for acne, and Exazel-N, for spot marks. I have been using these for the past six months but have not had many results and acne spots are not faded. I am 21 years old. Can I use a derma roller for my face? Can I shave my underarm hair? Please help.

Hi,

It would be great if you could send pictures of your face and axilla to assess the severity. I would suggest you start with the following:

  1. Tablet Azithral 250 twice a day for three days a week for three weeks.
  2. Apply Clindoxyl gel to the acne spots once a day for three hours only for two weeks.
  3. For the next two weeks apply Deriva CMS (Adapalene) gel to the acne spots for three hours.
  4. For black spots please apply Melaglow rich cream at night.
  5. For axilla, apply Candid gold powder in the morning, and Zole F cream in the night.
  6. Take a tablet of Alaspan (Loratadine) once a day at night for itching for 10 days.
  7. Drink lots of water, remove your hair.
  8. Derma roller cannot be advised for self-use. Please do not use it.

Thanks for sending the pictures. (attachment removed to protect patient identity). I would suggest you to:

  1. Drink lots of water.
  2. Do not meddle with the acne spots.
  3. Avoid using body spray directly on the axillary skin it will irritate more.
  4. Apply Zole F cream for 10 days only. Use powder twice daily after 10 days.

This is the age for getting acne as oil glands are active. So this problem can be controlled by the medication. If not to go for third-line medications like Isotretinoin.
