Hello doctor,

My fianceé had her last period a few days ago. My fianceé and I had physical intimacy but no intercourse, but she did not have her periods until now. She did a urine pregnancy test. It shows negative. We are very fearful after reading that pregnancy can happen even without intercourse. So she met a doctor yesterday and told only about the delayed periods and the doctor prescribed Deviry 10 mg for five days (taking for the last two days). But still she is tensed and so could you help us on the following:

  1. What may be the reason for the delay?
  2. Could you please tell us how we can 100 % confirm pregnancy?

I do not think you need to worry about pregnancy, as the urine test is negative. It detects hormone levels of above 25 mIU/ mL. But if you want to have a final confirmation the best method would be to have serum beta hCG (human chorionic gonadotropin). This will detect the smallest of pregnancy anywhere in the body. Even those less than 25 mIU/ mL can be detected. 100 % confirmation test. Pregnancy need not be the only reason for the delay in periods. Other possibilities are:

  1. Even the stress of thinking about the risk of pregnancy.
  2. Any sort of mental or physical stress, depression or anxiety or happiness, etc.
  3. Any sudden increase or decrease in weight.
  4. Usage of hormone pills to change the period date.
  5. Any recent fever or illnesses.
  6. Deviry (Medroxyprogesterone) will create withdrawal bleeding, so that is why your doctor has advised it.

The urine test is positive one day onwards after the expected date. Deviry (Medroxyprogesterone) does not affect Beta hCG (human chorionic gonadotropin). You can test it on any day. You will have your periods seven to ten days after stopping Deviry (Medroxyprogesterone acetate).

We did the serum beta HCG test, and the value was less than 1.2 mlU/mL. Five days course of Deviry medication was also completed the same day. But still, the period not happened. The last period starting date was last month. Could you please advise or suggest to us what may be the cause and what to do next?

Let me start by assuring you that once beta hCG (human chorionic gonadotropin) is negative on the blood test, there is nothing to worry about. Just relax. As you had already started on Deviry (Medroxyprogesterone acetate), I did not advise further on that. Many a time, the inner lining of the uterus may be thinned out, and the medication does not increase it. So bleeding may not occur. Also, the other causes I mentioned may not allow the endometrium to thicken. Just wait for a maximum of two weeks from the last pill; if still no periods, have a birth control pill after consulting a specialist and with their consent. No matter the cause, she will have periods within seven to ten days of the last pill.
