I have become paranoid for a few months that my coworkers are conspiring against me and serving as informants for the FBI. Occasionally I believe that I have psychic powers, reinforcing my belief about my coworkers. I know both are likely delusions, but that does not stop me from dwelling on them. Currently, I am taking Prozac 60 mg. What should I do? Kindly help.

Better assessment can be done if you answer the following questions. Do you hear any unknown or known voices talking to you about anything, commenting on your actions, or commanding you? Do you act upon those delusions? Does anyone in your family or close relatives have any symptoms or diagnoses of schizophrenia?

I do not hear any voices. I have become colder towards my coworkers and have tried to research to find out if the FBI truly is watching me. I have had suicidal thoughts due to these delusions. As far as I know, I do not have any relatives with schizophrenia.

The word "act," which I meant is if you ask your coworkers directly if they are conspiring with you or if you had an argument with them. Did any incident or stressful event happen at your office or home? May I also know why you have been taking Prozac (Fluoxetine)? Are you having any signs and symptoms of obsessive-compulsive disorder (OCD) or depression?

If no incidents prove the conspiracies, it is unlikely to be true. It might be delusions. I suggest you consult a psychiatrist for treatment of the same, and the addition of antipsychotics is needed for the treatment of delusions along with the Prozac you are already using. Moreover, you still have suicidal ideas because of these delusions. Hence I suggest you consult your psychiatrist in your region as soon as possible. According to all the symptoms you mentioned, it is depression with psychotic symptoms. Depression could also cause psychotic symptoms like delusions and even be associated with them. Schizophrenia could be a likely diagnosis if you have any auditory hallucinations.
