Hello doctor,

I had an implant in my mandible one week ago. Now, I have severe throbbing pain in my lower jaw radiating to multiple sites like adjacent teeth, lips, and chin. I take tablet Diclofenac 50 mg for pain. If I stop taking pain medicines, it hurts a lot. I am feeling compression. I do not have any history of neuropathy or other diseases.

I have attached my post-implant x-ray.

Kindly help.

I hope I could lessen your discomfort.

I have gone through your OPG (orthopantomogram) (attachments removed to protect the patient’s identity). There is a radiolucency that indicates a postoperative infection.

I suggest you take tablet Amoxicillin 500 mg twice daily for five days, tablet Metrogyl 400 mg (Metronidazole) twice daily for five days, and tablet Diclofenac as and when required. Do saline rinses every six hours a day for five days.

Infection.

Investigations to be done

X-ray (IOPA if possible).

Treatment plan

Tablet Amoxicillin 500 mg twice a day for five days. Tablet Metrogyl 400 mg (Metronidazole) twice daily for five days and tablet Diclofenac on SOS (when required).

Preventive measures

Saline rinses six hourly a day for five days.

Regarding follow up

Thank you for your reply.

I believe what you are referring to is a peri-apical lesion caused by a failed root canal treatment three years ago, and the tooth was removed a year ago. My dentist confirmed it to be healed and started with the implant procedures.

What should I do now?

To rule out the underlying lesion, I asked for an IOPA (intra-oral peri-apical radiograph).

Any way you take medication and update me for any problem. In addition, you can take multivitamins like tablet Zincostar or tablet Zincovit (vitamin A, B1, B2, B3, B5, B6, B12, C, Folic acid, Zinc, Magnesium, Manganese, Copper, Iodine, Selenium and grape seed extracts) once daily for 15 days.
