Hi doctor,

I am suffering from depression and I have symptoms of OCD for the past ten years. I repeat my activities like reading the same line again and again being scared of missing something. I count each and every number and believe four is to be my lucky number and even while breathing I count my breath. Sometimes, I hold my breath as I feel I am breathing in a wrong way. I am obsessed with washing hands, but I do not keep my room clean. I feel negative as nothing positive I can see or believe. I do not trust people. I feel something heavy in my heart and feel nervous the whole day. I want to know the reason for my nervousness. It is as same as we feel before the examination. I cannot focus on my work. I forget something and start doing other work. I think so much and live in a different imaginary world and so sometimes I cannot hear what others said to me. Earlier, I was planning to attempt a suicide. Now, I do not want to die, but still I am not enjoying my life. I feel my life is mechanical as I am not interested in daily chores or working at office. I am awake in the middle of the night and cannot sleep for 2 to 3 hours as I think so many unnecessary things of the past. I am going to get married in two months, but I am not happy with this relation. I choose to get married by my parent`s choice after I got rejected by my boyfriend. I am not having any friends and my relationship with my family is bitter nowadays. I want to feel fresh, excited and happy. Please suggest some medicine for me to feel happy and positive.

I can understand your concern. The symptoms you have mentioned suggest OCD (obsessive compulsive disorder). In more than 50% of patients, OCD depression is found to be comorbid. The symptoms like reading one line again and again and feelings that you might miss something are part of the obsession of doubts and compulsive checking. Repetitive hand washing is also a part of OCD due to the obsession of hygiene and compulsive hand washing. Persistent obsession and compulsion results in lack of focus. Low mood, reduced motivation and anxiety are seen, which ultimately complicate into depression. Lack of sleep, ruminations, poor focus and worries are also seen in OCD. Medicines like SSRIs (selective serotonin reuptake inhibitors) are the drugs of choice for OCD. SSRIs like Fluoxetine or Paroxetine can be used. Consult a psychiatrist, discuss with him or her and take the medicine with consent. Fluoxetine if used properly helps to reduce obsessive symptoms in most individuals. With proper treatment, your symptoms will start decreasing and you will be happy again. Do not feel that you are rejected. Everything happens for good and believe in yourself. Take care.

Can I have some guidelines on how to take a Fluoxetine? Is it safe to take this medicine? How many medicine should I consume a day and for how long should I take Fluoxetine? Is it true that the person who takes this medicine feel suicidal?

Fluoxetine should be taken in the morning after breakfast. The starting dose should be 20 mg after breakfast. In two weeks, the dose can be increased. The drug is a safe drug and will not increase the risk of suicidal ideas if taken under proper supervision of a doctor. For the best result, the drug should be taken for at least three months. Yes, it is normal to have increased restlessness and anxiety during first 10-12 days of starting Fluoxetine. As you have prescribed to me that after two weeks I can increase my dose to 40 mg, it has been 10 days of taking Prodep capsule IP 20 mg. I have asked in a market for 40 mg but not available, only 20 and 60 mg are available in market where I live. So my question is, can I take two capsules of 20 mg per day? While taking this medicine I feel my sensation are very low. Suddenly I am not reacting to the situation and in a group, I am quite silent. But, I feel relax. Though I am not feeling better, no more nervousness I feel. Still, I wake up every day at midnight and cannot sleep for hours. I strictly sleep at 9 and wake up at 7, maybe because I want to ignore talking to family members or other. I still feel sad and not feeling happiness around me. Do you think I should visit a doctor in my nearby place for proper consulting or I should wait for the better result? I am trying to keep myself busy. I feel alone but I do not want to mix with people. I do not know what is wrong with me. I feel isolated from family. In the office, things are better but I interact less with everyone. I hate when someone asked me why do you look sad. My problem is I am sad from last six months and I am not able to lift my mood. I really do not know I am exactly facing with depression or it is just my imagination. I was so happy to go lucky girl and it can be a pre-wedding nervousness also. I am a strong girl and it is the first time in life when I am not able to deal with a situation that is why I have thought of consulting Psychiatrist. Yes, the brand Prodep is not available in 40 mg dose. You can take 20 mg capsules twice a day, one in the morning and one in the afternoon. Avoid taking the drug in the evening. You are feeling relaxed, and that is a good thing. Wait for some more time to see the best result. The drug will control your bad mood. I do not think this is the time to visit a doctor. Wait for a couple of weeks. Do not think this thing is permanent. With proper medicines, you will again become happy go, lucky girl. I got married and recently came to know that my husband is also suffering from depression. He is on medicine for three years. His behavior is very dull towards me. He is secretive. He does not talk to me, only replies if I ask anything. He is always in a relaxing mode and not active at all. Either he sleeps or eats. The problem is I think he is addicted to opium. His eyes speak nothing and deep dullness in his eyes. There is no expression on the face and confidence level is very low. First of all, try to find out what kind of medicines he is taking. If you get a chance to find the prescription, then take a picture and upload the prescription. The medication can help in making his diagnosis. If he has burnt fingers or refuses to go to some relatives or friends place and likes to remain at home or suddenly disappears at a particular time of a day or gets very angry when he comes late or has silver foil like things in his pocket or black crude substance in pocket (if you find these) then these could denote he is taking some psychoactive substance of abuse. Most probably opium. If his pupils are pinpoint and he remains confused throughout a day then that could be also due to opium abuse. Other symptoms are chronic constipation, stained teeth, a weird smell from the body, slurred voice, etc. You are married for about one and a half months and no sexual relations are made. If he is not at all interested then try to talk with him. If he tells some problem then that should be corrected. If he does not take initiative then you should take initiative. Talk with him and then proceed further. He might be feeling shy due to lack of interaction between both of you.
