Patient's Query

Hello doctor,

I am a 43-year-old male with type 1 diabetes, which is generally well-controlled. I have experienced recurrent thrush infections about once or twice a year in the past, which typically clear up within a week. However, this year, I have been dealing with thrush for almost two months. I have been using Clotrimazole 1% cream, but it does not appear to be effective. Historically, I have had an issue where my foreskin becomes excessively tight, leading to discomfort, cracking, and bleeding when pulled back for cleaning.

My current medication regimen includes the following:

  1. Insulin

  2. Ramipril

  3. Bisoprolol

  4. Aspirin

  5. Atorvastatin

I am seeking guidance for this situation. Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I can understand your concern.

Based on your description and the photo (attachments removed to protect the patient’s identity) you have provided, it appears that you are dealing with balanoposthitis, a condition that is unfortunately quite common in diabetic men. I have one important question for you. Have you explored the use of steroid-based creams for treatment?

Steroid creams can be effective in reducing inflammation; however, they do come with the drawback of potential recurrence in a few months, and long-term use can lead to skin thinning.

In my opinion, if you are looking for a more permanent solution, I would recommend considering circumcision. Please share more details, and I will do my best to provide further assistance.

I hope this helps. Feel free to contact me if you have any further queries.

Thank you.

Patient's Query

Hello doctor,

Thank you for replying.

I have tried this cream. Is this a steroid cream?

Thank you.

Hi,

Welcome back to icliniq.com

No, that is not a steroid-based cream. I do not know where you are located and if you can get your hands on over-the-counter creams without a prescription. Try looking for a cream with the following content - Beclomethasone Neomycin. Clotrimazole is used to treat fungal infections. It would not offer you much relief (if at all). Please let me know if there is anything else I can help you with.

Thank you.
