Hello doctor,

My 10-year-old daughter weighs about 73 lbs. My wife accidentally gave my daughter one and a half Calpol 650 tablets three times a day (in a single day). We are also giving her the syrup Bro-Zedex about 7 ml. Is this regarded as a serious overdose? Should I be concerned? Are there any remedial measures? Please suggest.

Your daughter has been given a 450 mg overdose. Up to 2475 mg per day can be the maximum dose that can be allowable. So yes, she has been given an overdose of Paracetamol. I suggest you seek medical help from a nearby pediatrician. Paracetamol can affect the liver and kidneys. So, I would suggest you investigate her with serum glutamic-oxaloacetic transaminase (SGOT), serum glutamic-pyruvic transaminase (SGPT), prothrombin time (PT), serum bilirubin estimation and serum urea, and creatinine estimation for further workup after 24 hours of Paracetamol ingestion. Clinical watch up for the next 24 to 48 hours is necessary. Suppose any nausea, vomiting, abdominal pain, right upper abdomen pain, dark urine, yellow skin, or altered sensorium develops. In that case, Paracetamol overdose can be possible if she metabolizes Paracetamol overdose, then no need to worry.

Consult a pediatrician for examination and get done mentioned investigations as above. She has not been given a poisoning dose, which is usually above 7 grams. So, it is an overdose. So mostly, she will recover well without any signs and symptoms. But you cannot take the risk, so consult her with the pediatrician for an examination.
