Hello doctor,

I have been experiencing excessive dryness on my facial skin, coupled with the appearance of black tanned spots. After consulting with a local doctor, I was prescribed Photo ban 50 for this issue. Whenever I take medication, this dryness exacerbates. I have a history of endometriosis and was prescribed Dienegestrol and Ethinylestradiol. I suspect this medication is causing the tanning and dryness on my face. The skin around my face has become notably thin, delicate, and prone to cracking. While using the prescribed medication, my skin improves. However, once I discontinue it, the issues recur. I want to seek a proper diagnosis and effective treatment to prevent the problem from returning. I would appreciate recommendations for cost-effective creams, lotions, and affordable medication options. I am particularly concerned about cost, so please suggest budget-friendly solutions. I am looking for a balanced approach without excess. Also, I have noticed that my face has become pale, which is also a concern for me. Please help.

I have reviewed the details and images you provided (attachments removed to protect the patient's identity). Excessive skin dryness can result from using hormonal tablets and Fucidin H cream. If you are still using the mentioned cream, I recommend discontinuing it to prevent further dryness. Please consult the doctor who prescribed it for hormonal medication to discuss concerns about hyperpigmentation and dryness. The other medications from your doctor are beneficial for maintaining skin health.

I suggest the following treatment plan:

  1. Stop using face wash for three weeks until the dryness subsides. Cleanse your face with plain water during this time.

  2. Use Aha glow repair gel (botanical extracts) for two weeks to aid skin repair in the evening.

  3. Apply Angelskin cream (15 percent glycerin by weight) in the morning daily to extremely dry areas for hydration.

  4. I suggest taking one capsule of Evion 400 (Vitamin E) after dinner for two months to improve skin hydration.

  5. I suggest you consume one capsule of Omega 3 Gold (Fish oil 40 percent EPA, 30 percent DHA, gelatin capsule, fish, glycerin, purified water, and Alpha-tocopherol) after dinner for two months to enhance hydration.

  6. Ensure you drink at least 5 pints (2.5 liters) of water daily.

  7. Use UV Doux sunscreen gold SPF 55+ (C12-15 Alkyl Benzoate, Silicone Elastomer Blend, Octyl methoxycinnamate, Caprylyl methicone, Diethylhexyl butamido triazone, Diethylamino hydroxy benzoyl hexyl benzoate, Titanium dioxide, Zinc oxide, L-Ascorbic acid, Ethyl hydroxy stearate) every two hours between 9 am and 5 pm.

  8. Do not start using Melaglow-rich cream (Liquorice extract, pTero White, Arbutin, Niacinamide, and Kojic acid) now. Wait for three weeks until the dryness subsides, then apply it only on pigmented areas in a thin layer at night. Rinse it off in the morning.

  9. You can break a capsule Evion 400 and mix the oil with aloe vera gel for a face massage twice weekly. Leave it on for one hour and then wash it off to improve hydration and reduce pigmentation.

  10. Use coconut oil for face massage once every two weeks before washing with your current face wash.

Unfortunately, I stopped taking the medications I mentioned three months ago, and my face was fine. However, this skin issue resurfaced after starting the gynecologist's oral contraceptive pills (OCP). Please, doctor, consider reducing some of my medications, especially since I am already taking anti-anxiety pills. I have also discontinued the use of any face wash. Please help.

Taking capsule Evion 400 (Vitamin E) and capsule Omega 3 Gold Fish oil (40 percent EPA, 30 percent DHA, gelatin capsule, fish, glycerin, purified water, alpha-tocopherol) is not obligatory. They can assist in reducing dryness and pigmentation intensity, but it is entirely your choice whether to use them. You are free to proceed with the recommended creams. I suggest you use Ethiglo face wash (Kojic Acid 1 percent, Vitamin C 0.5 percent, Liquorice 1 percent, Nicotinamide 0.5 percent, Blend of AHA and BHA 1.5 percent, Triclosan, Vitamin E, D-Penthenol 0.5ercent) for cleansing your face, but please start using it only after a three week waiting period. Consult your doctor and take the medicines accordingly.

Ahaglow repair gel (botanical extracts) should be used for two weeks to soothe the skin. Angelskin cream (15 percent Glycerin) can be continued on dry areas as a moisturizer because it provides excellent moisture. Melaglow-rich cream (Liquorice extract, pTero White, Arbutin, Niacinamide, and Kojic acid) should only be started after three weeks and applied in a thin layer on pigmented areas. Both Angelskin and Melaglow-rich creams can be used for an extended period, even up to 8 to 12 months.

This cleanser effectively cleanses the scalp without causing dryness. Please remember to apply the cleanser only to the scalp, avoiding the hair. I also suggest you use Triclenz cleanser. Use 3 to 4 drops on a wet scalp, apply, massage for one minute, leave it on for two minutes, and then rinse off. For your hair strands, you can use any hair conditioner like triflow conditioner. Apply it after shampooing, focusing on the hair strands, massage for 1 minute, leave it on for 3 minutes, and then rinse.

No. There is no side effect. You may continue ketoconazole shampoo once a week and triclenz cleanser once week each with 3 days interval in between both the shampoo applications.
