Hello doctor,

I have an ear infection, and my ear is swollen and blocked. I have prescribed Amoxicillin for one week, but it did not work. I could not consult a doctor in person because of the pandemic. What should I do now? Kindly help.

Thanks for the briefing regarding the problem in concern. I had a detailed look at the picture you have sent (attachments removed to protect the patient’s identity). There are two possibilities in your case. 1) Fungal infection involving the middle ear canal, what we call otomycosis. 2) Infection of the hair follicle in the outer ear canal (hair-bearing zone). We call it folliculitis. Both these conditions come under one category called otitis externa. Amoxicillin is usually not the drug of choice for otitis externa, which is probably why you did not have many benefits. It is advisable to take fluoroquinolones for best results. Kindly consider the following advice; 1) Take tablet Ciplox 750 mg (Ciprofloxacin) (500 mg + 250mg) one tablet two times a day after food for one week. 2) Take tablet Zerodol-P (Aceclofenac and Paracetamol) one tablet two times a day after food for one week. 3) Take tablet Ultracet (Acetaminophen and Tramadol) one tablet at night after food for one week. 4) Tablet Pan-D (Pantoprazole and Domperidone) one tablet once in the morning before breakfast for one week. 5) Keep the ear dry and refrain from using earbuds. 6) You can use antifungal ear drops containing Clotrimazole and Lidocaine, three drops thrice daily for five days. 7) You can smear a good antibiotic like Soframycin over sterile cotton and apply it on the outer surface of the affected ear and repeat the process every six hours for the next ten days. 8) Kindly check your blood sugar. In otitis externa, you will develop very excruciating pain in the ear, which resolves with anti-inflammatory and painkillers. This typically takes around three days for things to heal if this antibiotic has worked for you. The feeling of fullness is due to swelling in the ear canal, and it will slowly resolve with time. You might also observe some discharge in the canal that has come out from the small abscesses formed due to the folliculitis. This is an extremely common entity, and there is absolutely nothing too serious out there. You dont have to worry as things should be fine in a few days. Please do take adequate care. Warm regards.
