Why do I have ED and testicle sensation loss after injury?

Hello doctor,

Last year, a boy pressed my penis, and after that, I started having erectile dysfunction (ED) problems. I took some medicine, and after seven days, I noticed my ED improved slightly, but it has not fully recovered. My left testis was swelling, so I took medicine and the swelling went away, but now my left testis feels hard, small, and sometimes painful.

I visited another doctor who prescribed Tadalafil and recommended a testosterone test, which came back normal. My ultrasound (USG) and urine test were also normal. However, for the past five months, I have been feeling reduced sensation in both testicles. When I tap them, I can feel it, but with a light touch, I feel almost nothing. Currently, I have mild erectile dysfunction. How can I manage this?

You have noticed erectile dysfunction (ED) after your penis was pressed. Later, you developed swelling of the left testis, which improved with medication, but you still felt the testis was hard, soft, or small at times. You were on Tadalafil, and your testosterone levels, ultrasound, and urine tests are normal. However, you still have mild ED and reduced sensation in both tests for five months.

I would suggest the following measures based on your condition:

Normal ultrasound, hormone tests, and urine tests are very reassuring. If there were a major injury to the testicles or nerves, ultrasound and lab tests would usually show abnormalities.

The testes themselves do not have strong surface sensitivity like the skin. They feel pressure but do not fine-touch well, which is normal. If sensation truly feels reduced compared to before, it may be due to temporary nerve irritation from the swelling or inflammation you experienced. Nerve healing can take several months.

Mild erectile dysfunction, since you can still get erections with masturbation and Tadalafil works, your blood flow and testosterone are fine. These points more toward psychological causes (performance anxiety, trauma from the pressing incident), along with possible temporary nerve sensitivity changes.

Stop worrying about permanent damage; your tests are normal. Continue Tadalafil if your doctor has prescribed it, as it is safe for short-term support. Give your nerves time; nerve recovery can take six to 12 months. Avoid constant checking or tapping; this increases anxiety and makes you focus more on the problem.

Maintain a healthy lifestyle by:

  • Regular exercise.

  • Sufficient sleep.

  • Avoid smoking and alcohol.

  • Manage stress well.

Reduce porn and over-masturbation as they can blunt sensitivity and worsen ED.
