Hi doctor,

This is a picture of my 25-year-old daughter's eyes. She is studying, and she is concerned that one corner of her eye has recently turned brownish (the picture is of both eyes for comparison). She says it is a little irritated, and this has appeared in the last few weeks. She started using topical Tretinoin about the time she noticed it. She says she is too busy to go to an ophthalmologist at this time. She sent me this to see what I thought. Please let me know what you think. Thank you.

I thoroughly read your query and understand your concern.

There are a few possibilities after I went through the picture (the attachment is hidden to protect the patient's identity). This comes under conjunctival pigmentation, and there are a few reasons for this pigmentation which can ne complexion-related pigmentation, naevus and melanoma (rare). My suggestion now is it is complexion-related pigmentation, and no treatment is required. If it is naevus, it is also benign and does not need treatment. However, we need to observe it. If there is an increase in size or it gains height (becomes elevated), increases in the density of pigmentation, or large vessels start developing around it or in it, then an ophthalmologist consultation is suggested. Melanoma is very rare. Now, coming to the present condition, it seems like a benign pigmentation as it is flat, small, and lightly pigmented, and there is no large vessel feeding the lesion, so in my opinion, no treatment is required at this moment.
