Hi doctor,

I have had right flank pain that comes and goes for about ten weeks. It is dull and moves around my right flank area, and sometimes it radiates to the back and lower abdomen. A few times, it wakes me up at night and is relieved by urinating. I have a history of breast cancer and thyroid cancer. I am currently taking tablet Synthroid and tablet Lipitor. What to do?

It would be best to take a computed tomography (CT) scan of the KUB (kidney, ureter, and bladder) region to rule out any small renal or ureteric calculus (the presence or formation of stones within the ureters).

The attached ultrasound shows a 0.15 inch renal calculus in the mid-pole of the right kidney. Could this be the reason for causing my pain?

It is unlikely. Generally, renal calculus does not cause pain unless it obstructs urine flow in the ureter. However, renal or ureteric calculus diagnosis sensitivity is just 60 to 70 percent in good hands. So it can miss many things. So it is better to get computed tomography (CT) scan done.

I will take a CT scan. Anything specific, for example, with or without dye? My pain is in the flank area between the right lowest rib and the top of the pelvic bone. It spreads a little towards the back and sometimes to the abdomen or groin. Is there a specific area for renal stone pain?

Yes, obstruction of kidney either because of ureteropelvic junction obstruction or stricture. However, ultrasound does not show any signs of hydronephrosis (the swelling of a kidney caused by a build-up of urine). So stricture is unlikely the cause, but ureteropelvic junction blockage is possible.

I looked over the image (attachment removed to protect the patient's identity). You can feel it. It is a hard stone. I suggest you perform a chemical analysis to determine its composition. Are you relieved after passing these?

It feels hard and firm. I can reach out to my doctor's office to ask about composition. How quickly should I get it analyzed? I still feel sore on my flank and groin, but my ache has reduced. I am not as sore as I was 24 hours ago.
