Hello doctor,

My husband is 30 years old and has had consistently foamy urine for the last five months. We have been to different doctors and had several CBC and kidney function tests done and everything appears normal. He does not have any other symptoms. We have noticed when he eats less protein, the bubbles are reduced however this is not for every time. What could this be? I have attached his most recent blood and urine tests taken in the last two months. Any insight you can offer would be appreciated.

I understand your concern regarding froth coming out in your husband's urine. I have gone through your reports (attachment removed to protect the patient's identity) and I can see the urine albumin and urine albumin creatinine ratio are within normal limits. So one thing is clear this is not because of any kidney pathology. There are a few other causes of frothy urine like a fast flow of urine, exercise, urine infections, or a fistula between the colon and ureter. But these are associated with their respective symptoms. Another issue can be spermaturia, where sperm can come out in urine due to retrograde ejaculation into the urine bladder.

Hello doctor,

He does not have any other symptoms. It is happening on a daily basis. The bubbles take a long time to disappear and recently there has been a moldy smell. It tends to happen more towards this evening and there are some nights where my husband will wake many times to urinate. Could this help narrow down what might be affecting my husband? Thank you for clarifying that it is not a kidney problem. We are still confused about the whole thing, to be honest, but it is good to know that we do not need to be worrying about his kidney. Is there any advice you can give in how we should proceed?

Moldy odor can be because of a urine infection. But as you said he has no symptoms. Does he have any fertility issues? Is there any history of abdominal pain, not able to control urine, or having loose stools?

Hello doctor,

In the most recent weeks, he had some abdominal pain but that has faded since he started drinking probiotics. He can control his urine and his stool has been normal. He has occasional diarrhea. We do not know if he has any fertility issues. What could this mean?

If you have kids, then he has no fertility issue or if you are trying to conceive but that is not happening, then fertility issues can be considered. Yes if he has burning during micturition and he drinks less water per day, then urine infection is likely. And that may cause frothy urine.

Thank you for your response. Why would a fertility issue cause foamy urine? I really appreciate the time you have taken to answer my questions. I have attached today’s urine photo. We do not think it is an infection because it has been a long-standing issue. What course of action would you advise taking? The burning sensation only happens rarely, most of the time the only issue with his urine is the foam.

This much water intake makes the urine concentrated, which leads to frothy urine. Just like if you pour any concentrated solution in the water and it would produce froth, the same thing is happening here. Ask him to drink around 2 to 2.5 liters of water per day and then look for the froth.
