Hello doctor,

I am a 24-year-old male, and I have noticed a dark red spot underside of my foreskin that has been there for some time. After cleaning it thoroughly, I felt a slight tingling sensation, but it subsided. This spot is only present in one area, and there are no signs of a rash or anything similar in any other area. It appears relatively small and slightly swollen. I have had this for over a year, but it does not seem like an STD. Please help.

I have read your query and understand your concern.

It would greatly help if you share a picture of the affected area. If you have had unprotected sex, it is important to consider the possibility of a sexually transmitted infection. I suggest you undergo TPHA (treponema pallidum hemagglutination), HPV DNA PCR (human papillomavirus DNA polymerase chain reaction) test, herpes simplex virus (HSV) type 1 and type 2 IgG, herpes simplex virus (HSV) type 1 and type 2 IgM (serum), chlamydia trachomatis IgG, HIV (human immunodeficiency virus) type 1 and type 2 antibodies, Screening Test HBsAg (hepatitis B surface antigen) and a complete urine test.

For treatment, I suggest the following:

  1. If unprotected sex was not a factor, it could be a simple bacterial infection. In such a case, I suggest using T-bact (Mupirocin two percent) ointment for local application and taking tablet Doxycycline 100 mg twice daily for five days.

  2. If there is itching, consider the possibility of a fungal infection. I suggest you use Candid B ointment (Beclomethasone and Clotrimazole) for local application and take the tablet Cetzin (Cetirizine) once a night for itching relief. Consult your doctor and take the medicines accordingly.

  3. Ensure that you wash your genitalia twice a day, in the morning and at night, using normal water (neither hot nor cold).

  4. Avoid using heavy pH soap.
