Hi doctor,
I am a 75-year-old. I have had type 2 diabetes for the past ten years and hypothyroidism for the past eight years. Mild hypertension has been present for the past 40 years, and cervical spondylosis for a few years. Benign prostatic hyperplasia (BPH) has been present for eight years. I take medicines regularly for all these ailments. All are under control as of now. I have no problem with them so far. I even walk approximately four kilometers every day. Whenever I feel like reeling, I wear a collar for some time, and it subsides.
I am on medications like tablet Betacard (Atenolol) 50 mg, tablet Amtas (Amlodipine) 10 mg, tablet Glycomet SR (Metformin Hydrochloride) 500 mg twice a day, tablet Thyronorm (Thyroxine Sodium) 50 mcg, capsule Tamdura (Tamsulosin and Dutasteride) 0.4 mg, tablet Deplatt (Clopidogrel and Aspirin).
Of late, I am experiencing reeling for three to six seconds when I lie down on the bed or when I am getting up from the bed. The duration of this reeling-like sensation is concise, say less than ten seconds. Also, on my left forehand, I sometimes feel the sensation of crawling ants under the skin, and the burning of the forehand is present at times, often associated with pain and itching. I am now taking tablet Methylcobalamin (Vitamin B12) every day as per my doctor's advice. Right now, I am experiencing a frozen shoulder of the left hand.
Kindly evaluate my case and advise me of the reason for the momentary reeling, pain, itching, and crawling ants sensation.

Sorry to hear about your symptoms. I read your query, and I understand your concern.
The momentary reeling you experience is because of the change in position or posture and is called positional vertigo. Does this reeling occur every time you change sides on the bed?
If yes, I suggest you take (consult with your specialist doctor, discuss with him or her and take medicine with their consent) tablet Vertin (Betahistinedemesytate) 16 mg twice daily for five days and 8 mg twice daily for the next five days and then stop taking it. Then consult with an ENT doctor for a hallpike maneuver to correct the benign paroxysmal positional vertigo (BPPV). Take Micro-D3 60000 (Cholecalciferol) IU once a week for eight weeks.

Thank you so much for your reply.
I have had BPPV for many years. I take tablet Vertin (Betahistinedemesytate) 16 mg whenever required. I always sleep on the right side because I have meniere's disease in the left ear. This is why I always get up from the bed, keeping my right shoulder in support of my head, and then I do not get vertigo. I took an MRI of the brain five years back for this condition, and its results were normal.
Can you please advise me on the left shoulder problem? Why do I have crawling ants sensation, pain, and itching?

Tingling numbness in the shoulder region is mainly because of a few reasons like cervical spondylosis and vitamin D deficiency. When did you get your last magnetic resonance imaging (MRI) of the spine done? Is the crawling sensation present all day? Is there any other complaint like weakness of the left shoulder or twitching of muscles of the left side?
Please send your magnetic resonance imaging (MRI) report of the cervical spine if possible.
