Why did my penis swell after applying Clotrimazole cream?

I am suffering from itching in my thighs, penis skin, and scrotal areas. I thought it was a fungal infection, and I purchased Clotrimazole cream 1%. After applying it at night, the next day I noticed that my penis was swollen on one side, with no pain. Is this because of the cream, and has it still not returned to normal? What should I do now?

The surface looks dry and wrinkled rather than inflamed. This appearance is not typical of a fungal infection.
This swelling is very likely an acute allergic or irritant contact dermatitis or an angioedema-like reaction caused by Clotrimazole cream, especially since the swelling appeared the very next day, is one-sided, and occurred without pain, fever, discharge, or ulcers. These features strongly suggest a local allergic or irritant reaction rather than an infection.
Genital skin is extremely sensitive, and in some people, Clotrimazole can trigger significant swelling, particularly on penile skin. This reaction can look alarming, but it is common and does not mean anything serious has happened.
It is important to reassure you that this is not permanent damage, it is not sexually transmitted, and it is not dangerous unless it is associated with increasing pain, marked color change, or difficulty passing urine, none of which you are experiencing at present. The appearance is consistent with localized edema due to skin irritation.
You should stop Clotrimazole immediately and avoid applying it again. Gently rinse the area with plain water only, without soap, antiseptics, or scrubs. Keep the area dry, loosely covered, and avoid tight underwear or friction until the swelling settles.
At this stage, apply only a bland emollient such as plain petroleum jelly twice daily to help repair the skin barrier. If itching is troublesome, you can take a tablet Cetirizine 10 milligrams at bedtime, and twice a day in case of severe itching. It can help reduce both itching and swelling. Do not apply any powders, antiseptics, or home remedies for now, as these can worsen irritation.
To manage the swelling and the reaction, apply one percent Hydrocortisone cream at bedtime only on the affected area for at least five to seven days, and then stop it.
Typically, the swelling should start to reduce within 48 to 72 hours, with near-complete resolution over five to seven days. However, you should seek urgent medical care if the swelling rapidly worsens, the skin becomes bluish or black, or if pain, fever, discharge, or difficulty urinating develops.
