Hi doctor,

I recently had an upper gastroscopy and was told I have a shallow linear ulcer, a small hatial hernia, and acute gastritis. I have been taking tablet Esomeprazole every day and have had no symptom relief, and I am getting worse. My current symptoms are:

I intend to find out if the cause of my symptoms from acute gastritis, partial hernia, and stomach ulcer is related to SIBO (small intestinal bacterial overgrowth). My symptoms are as follows:

  • Weight loss.
  • Stabbing pain in the chest/heart when trying to take a deep breath.
  • Shortness of breath after eating and drinking water. Unable to drink much water because of the shortness of breath (pain going into the upper back)
  • I am malnourished and have shortness of breath.
  • Bloating.
  • Burping constantly.
  • Unable to sleep in the past two weeks. Lying down makes me feel like I can not breathe.
  • Constipation (daily) unable to empty bowel.
  • Upper abdominal pain.
  • Nausea, which gets worse after lying down.
  • Throat pain (trouble swallowing), reflux.
  • Tingling in hands, feet, and arms.
  • Urinary incompetence.
  • Increased bruising.
  • Heart palpitations.
  • Dizziness.
  • Oral Trush.
  • Dehydration.

There are specific issues that should be addressed.You had significant weight loss, which cannot be ignored, and you should be appropriately evaluated for weight loss, not just focus on your stomach.Oral thrush is not common in a young male with normal immunity; again needs detailed history, physical examination, and laboratory investigations, including blood tests.Severe chest pain and shortness of breath during drinking and eating are also not expected. Chest X-ray and repeat endoscopy can be considered.Urine inconvenience again is not explained at your age and needs evaluation.Dizziness and increased bruising can be a sign of vitamin and micronutrient deficiency.My suggestions are as follows:

  • Detailed history and physical examination, baseline investigations including complete blood profile, serum protein, calcium, and tests for malabsorption.It would help if you first visited an internist (internal medicine) consultation. He will be the person to direct you to the appropriate doctor or any other specialist.For now, you should continue Proton pump inhibitors (acid suppression) medicine and consider repeat endoscopy as it was significant gastritis on the previous one.
  • Should improve your diet, a home-cooked balanced diet.
  • No alcohol or drug abuse, if any.
  • You can take a high-fiber diet or laxatives like Polyethylene glycol sachet once or twice daily for constipation.
