May I know what the query is regarding? It seems that the person you are concerned is pregnant. Routine antenatal investigations are needed. An early scan at around seven weeks of pregnancy is also warranted.

Yes, the query is regarding my wife. Her LMP was due seven days back. However, she experienced excruciating lower abdominal pain yesterday and spotting a few hours later. She took a pregnancy test and it was positive. What could really be going on?

If she has excruciating lower abdominal pain with spotting, it could be an ectopic pregnancy which is an emergency. She needs to be seen by a gynecologist to rule out the same as tubal ectopic pregnancy may rupture and cause bleeding inside the abdomen which could cause the patient to land up in a shock. So, kindly visit your gynecologist and get an emergency scan done for locating the site of pregnancy.

It was a one-time pain yesterday. Also, I forgot to mention that the pregnancy test was negative as of the day of LMP. A day before that she said she saw her period really confusing though. Are you suggesting the scan and consultation with a gynecologist? Should the ultrasound scan be transvaginal or transabdominal?

It should be a transvaginal scan only in the early trimester. I would still advise you to pay a visit to your gynecologist once.

Yes, I went through the scan report (attachment removed to protect patient identity). It does suggest a very early pregnancy probably in the cervix. Cervical pregnancy can cause torrential life-threatening bleeding. I would advise you to keep following up with your gynecologist regarding a follow-up scan to confirm it and to have the necessary treatment.
