Hello doctor,

I have had GERD for the last 10 years. Last year, I was diagnosed with H. pylori and medicated with triple therapy. For the last 15 to 20 days, I have been having symptoms of GERD along with new symptoms of tightness below the ribs, they did an endoscopy and biopsy and said H. pylori is still there. I do not feel my symptoms are the same as H. pylori when compared to last year. I have attached the reports of endoscopy and biopsy. Are the reports confirming that I have H. pylori? Are there any better tests to confirm? If yes, what should I do now?

Kindly help.

I have reviewed your case history and endoscopy report (attachment removed to protect the patient's identity) along with the biopsy report. So, your endoscopy report is grossly normal, with no major ulcers or any obstruction in the food pipe; a small hiatal hernia is common at this age, not explaining your current symptoms such as there is H. pylori infection on your biopsy. It is not necessary that it always causes similar symptoms. Otherwise, it is again a very common infection in this part of the world. A biopsy is one of the best tests to confirm H. pylori. If chest tightness increases with walking, one should check cardiac status as well. Heart-related problems must be ruled out by taking certain tests like ECGs (electrocardiogram), etc.

Thank you for the reply

I have done the ECG, and it is fine. I do not have tightness in my chest. The tightness is present below the ribs and upper abdomen on the left side. Due to this, I am not able to sleep well (just managing four hours), and it is turning out to be like anxiety or stress as well. As I said, I have GERD symptoms as well; what should I do? Do I need to take medication for H. pylori eradication or tightness?

H. pylori does not cause symptoms that you are experiencing these days, especially in the presence of almost normal endoscopy. Your ECG is normal, which is reassuring. I hope you have done your ultrasound abdomen as well; if it is normal too, then this pain might be related to muscular origin or sometimes exacerbated by anxiety or stress. H. pylori is on biopsy; one can take medicine, but one cannot ignore H. pylori on biopsy. Simultaneously, try to avoid stressors and anxiety.

Thank you for your reply

I also had an ultrasound of the abdomen done along with some other tests, and it is normal. I do have acidity, burping, and gas as well, but they are severe. I get these symptoms during anxiety or stress, which I have been in for the last 30 days with very little sleep. Can you suggest if I need to take medicines for H. pylori?

With normal endoscopy, I feel H. pylori treatment can be delayed because your symptoms are not typical for that. It can be treated later if your symptoms are persistent. However, your anxiety or stress should be treated. It will significantly improve your symptoms.
