Hello doctor,

My mother is 76 years old and has been suffering from pain in her thyroid gland due to goiter for the last three days. The FNAC is benign and not malignant. The blood test and thyroid function tests are normal.

I read your query; it is certainly a benign nodule, according to the attachment (attachment removed to protect the patient's identity). If it is causing her pain, it can be removed surgically, or the cyst can be aspirated or ablated. However, considering her age, you will need an endocrine surgeon to review her suitability for any of these procedures. It is alright to leave it as it is, provided a regular ultrasound thyroid and thyroid function test is carried out every six months. You can give her simple analgesics like Paracetamol (Acetaminophen), one gram orally once or twice a day after consulting a specialist doctor. We do not want to give her very strong painkillers as they have their side effects.

Thank you for the reply. Is there anything serious you feel about this pain that could be a cancer symptom or anything else? We will get the required tests done. Please suggest what we should do next because when we consulted the doctor, they said to remove that goiter. My mom is very weak and is 76 years old, so she can not bear that kind of surgery. So please suggest what can be done next.

Do not worry; the cyst looks benign. However, I recommend getting it removed if your mother is fit. But you can still consult an endocrine surgeon to know if there is an alternative technique to help her. They will check her fitness before doing anything invasive.
