Hello doctor,

I have had continuous headaches for the past three months. I underwent an MRI scan, and the reports were normal. The doctor told me it was a migraine, but tablets did not relieve the headache.

Currently, I am on the following tablets,

  1. Amitone plus.
  2. Ciplar LA - 20.
  3. Sibelium - 5.
  4. Pantosec - 40
  5. Naxdom - 500.

  1. Since when did you start to have these headaches? Is it since three months only or they used to occur intermittently before also?

  3. What kind of headaches do you feel like heaviness, bursting, squeezing, stretching, throbbing, or some other type?
  4. Are your headaches intermittent or persistent?
  5. If intermittent, one episode of headache lasts for how much duration (without pain killer tablet)?
  6. Do you feel any nausea or vomiting during headache episodes?
  7. Is headache associated with any redness in the eyes, watering from the eyes, or nasal blockage?
  8. Have you noticed any specific trigger factor for this headache till now?
  9. How frequent are these headaches? How many times in a week do they occur?
  10. For how long have you started taking Amitone (Amitriptyline), Ciplar LA (Propranolol) - 20, and Sibelium (Flunarizine) - 5?
  11. How is your vision for distant and near objects?

Other information:

  • How is your sleep routine? Do you sleep sooner or later after lying in bed? Is your sleep disturbed? Do you feel fresh on awakening from sleep in the morning?
  • What is your usual mood in daily activities? Do you think of past or future things?

Preventive measures

The medicines you mentioned are used for prevention (to reduce the frequency and intensity of future migraine headaches. And they may take some time (a few weeks) to show their results. The usual approach for migraine prevention is that if the headache remains the same until that time or any side effects appear, then the dosage or medicines are modified.

Thank you for the reply.

My headache has been continuous for the past three months, and the pain stays the same. The pain is in the front and the back part. The pain is persistent for a day long. My sleep is proper for 8 hours, but I take tension even for simple things. I have been taking the tablets for five days.

Your description is very short and not detailed as I tried to ask for. As the MRI reports are normal, your condition looks like a tension headache.

I suggest,

Tablet Amitone (Amitriptyline) may be beneficial in it.

The effect of the medicine may start from 10 to 14 days. Based on the response with a dose, the next dose is decided. Also, I would suggest you consult an opthalmologist to look for refraction, intraocular pressure, and fundus. Along with this, try to start an exercise program like jogging for 20 to 30 minutes in the morning.

Regarding follow up

Please mention doses of the currently taking medicines and will review them after two weeks.
