Hello doctor,

Last week, during the night, my relative developed chest pain and felt very uncomfortable. He was taken to the hospital, and after tests and preliminary medications, he was advised to undergo PTCA (percutaneous transluminal coronary angioplasty). He is 51 years old and has a history of diabetes. The reports are enclosed.

Please advise whether he requires angioplasty or coronary bypass surgery.

The medication he is receiving is appropriate and should be continued until bypass surgery. If angioplasty is performed within 24 hours of the attack, it can be beneficial for the patient, as it can be done on an urgent basis; the shorter the time interval between the attack and either procedure, the greater the benefit. This benefit typically lasts for 24 to 48 hours.

However, since this crucial time period has already passed, there is no urgency now. He should undergo bypass surgery, as he has too many blockages, and stenting will not be feasible in all these areas. Please discuss this with the cardiac surgeon regarding the bypass, which should be performed as early as possible for the best results.
