Hello doctor,

I am a 32-year-old man. About two years ago, I was using a dildo (male) and ended up having old brown blood in my semen for three months. I consulted various doctors, and they just asked me to give it time. It went away after three months, but I have been having enlarged prostate symptoms even though my doctor said it is normal. So about two years later, I was doing squats from a workout with lightweight and had fresh blood in my semen right afterward. But ever since that happened, small amounts of fresh blood are seen in the semen more often without working out and are slightly inflamed. The urine stream is slower than it used to be. Maybe when I damaged something in there, some scar tissue grew and might be putting pressure on something. I have only had pain two times ever. Right after the squats and orgasm, it had an injury because the pelvic area was inflamed and tight. It was uncomfortable for about a week, but it was fine later. Recently after orgasm there was a pain in the bladder prostate area twice. After ejaculating, I usually feel slightly inflamed for an hour, and some semen with some blood comes out after 30 minutes. I have had blood tests, and they are fine. I had a recent prostate exam unrelated to this, and the doctor said it felt fine. I am of normal weight. I have not done any anal stuff since. What do you think the problem could be?

It looks like hematospermia due to inflammation, infection, or tumor. Proper investigations are required to diagnose the condition. Inflammatory causes are infections, stones, and cysts present with discomfort or dysuria, or hematospermia. Investigations required for semen culture include,transrectal ultrasound and pelvic MRI (magnetic resonance imaging). If the cause is an infection, it can be treated with medications.
