I understand your concern.

I have reviewed your history and picture (attachment removed to protect patient identity). It seems like a sentinel pile. They are not piles but an anal fissure that has been present for so long, leading to a bulging appearance resembling a pile. Well, the treatment you are on is majority Ayurvedic, but there are available allopathic medicines that can be used instead. I would give you some therapy, but I want you to be examined in person by a local general surgeon if you need surgical removal. Otherwise, at the moment, take the following treatment.

  1. Ispaghula husk two tablespoons daily with water, regardless of whether you have soft or hard stools. Fiber has to do with many other things in your gut besides the stool-softening effect.

  2. Ciprofloxacin 500 mg (antibiotic) twice daily for five days.

  3. Metronidazole 400 mg (anti-anaerobic) thrice daily for five days (can cause some gut upset and metallic taste in the mouth but has a relatively low side effect profile).

  4. Sitz bath. Put approximately 5 ml of Chloroxylenol antiseptic solution in a bathtub filled almost half with tap water and at least try it thrice daily.

  5. Keep your area clean near your bottom.

  6. Remove hair.

  7. Reduce weight. Increasing weight would impair your defecation (stooling) mechanism, and that would create abnormally high pressure over your bottom. This worsens anal fissures in the long term. Let me know in one week if symptoms improve.
