I am experiencing pain between my shoulder blades, around my spine, and I also have anxiety and a hiatal hernia. I engage in smoking and drinking over the weekends. I kindly request your suggestions on what I should do.

I read your query and understood your concern. Based on the images and report you provided (attachment removed to protect patient's identity), your shoulder pain seems to be caused by trapezitis. I suggest you follow the instructions mentioned below:

  1. Avoid lifting heavy weights.
  2. Avoid arching your back, instead, bend your knees to pick up something from the floor.
  3. Avoid sitting for long hours.
  4. Avoid riding a two-wheeler.
  5. Improve your posture while sitting. Try to sit up with your back straight.
  6. Try to get up from your chair every 20 minutes and take a quick walk around before continuing work. Always sit on a chair with back support.
  7. Do not get up suddenly from a sleeping position. Rather, turn to the side and then get up.
  8. Take bed rest only if experiencing severe pain.
  9. Apply heat pad locally over the affected area at least twice daily.
  10. Apply gel containing Diclofenac, Methyl Salicylate, and Menthol locally twice a day.

I also suggest you to take the following medications:

  1. A tablet containing Pregabalin and Methylcobalamin 75 mg at night for 15 days before food at seven o'clock every day.
  2. A tablet of Etoricoxib (selective COX-2 inhibitor) 60 mg two times a day for ten days after food.
  3. A tablet of Rabeprazole (Proton pump inhibitor) 20 mg two times a day for ten days before food.
  4. A tablet containing Calcium and vitamin D3 once a day for 30 days before food.
  5. A tablet containing Cholecalciferol 60000 IU once a day for ten days, then once a month for 11 months.

Take these medications only if you are not allergic to them and if you are not a known case of bronchial asthma. I suggest you consult a specialist, talk with them and take the medications with their consent. I also suggest you do physiotherapy exercises, including core strengthening exercises and start training under a physiotherapist. Physiotherapy should be started once the pain reduces. Please note that jogging, running, and weight-lifting may aggravate back problems. I suggest you perform the exercises mentioned below:

  1. Neck rotations.
  2. Shoulder shrugs and shoulder rotations.
