Hello doctor,

As I am going through open heart surgery, my blood pressure and sugar levels are not getting controlled. As a result, I am facing high creatinine, eGFR (estimated glomerular filtration rate) levels are low, and I am an alcoholic and smoker. Currently, I am taking Lantus injection 30 units, tablets Invokana 100 mg, Diamicron XR 60 mg, Volibo 0.3 mg, Rozucor ASP 20 mg, Cilacar 10 mg, and Concor 5 mg. Please help.

I am very surprised that you have been smoking and consuming alcohol even after open heart surgery. You have uncontrolled Hba1c (blood sugar), high hemoglobin due to smoking, and high triglyceride levels due to alcohol consumption.

First of all, I need an ultrasound of your abdomen to check for kidney size and prostate size. I would also recommend adding the tablet Trajenta 5 mg to your regimen and increasing Lantus injection by two units per day till you reach desired levels. However, please consult a physician, discuss with them, and start taking the medications with their consent.

I want to see your ultrasound report and know how many cigarettes you consume and how much alcohol you take daily.
